Analysis of Risk Factors for Colorectal Adenoma Complicated With High-Grade Intraepithelial Neoplasia
Objective To explore the independent risk factors of colorectal adenoma(CRA)complicated with high-grade intraepithelial neoplasia(HGIN).Methods A total of 256 inpatients who underwent total colonoscopy at the digestive endoscopy center,Fujian Provincial Hospital South Branch from January to December 2022 and whose pathological results were confirmed to be CRA were selected.Basic information of patients was analyzed,including gender,age,body mass index(BMI),relevant underlying medical history,polyp location and morphology,post-polypectomy pathology information,etc.Univariate and multivariate logistic regression were used to analyze the risk factors of CRA complicated with HGIN.Results Univariate logistic regression analysis revealed that age had a statistically significant impact on the development of CRA complicated with HGIN(OR=1.037,95%CI 1.006-1.069,P=0.019).The effect of the number of adenomas on CRA complicated with HGIN was statistically significant(OR=0.188,95%CI 0.065-0.543,P<0.001).The effects of adenoma morphology,pathological tissue type,adenoma location,adenoma morphology and mucosal surface on CRA complicated with HGIN were statistically significant(P<0.001).Multivariate logistic regression analysis found that advanced age,adenomas with villous components in pathological type,and rough or ulcerated mucosal surface were highly correlated with the incidence of CRA complicated with HGIN(P<0.05).Conclusion Advanced age,adenomas with villous components in pathological type,and rough or ulcerated mucosal surface are independent risk factors for the development of CRA complicated with HGIN.
