软式内镜高水平消毒后储存期安全性的实践研究

目的 通过监测高水平消毒后软式内镜存放于镜柜不同时间时内镜管腔的微生物污染情况,以探讨软式内镜的安全储存时间。方法 使用随机抽样法选取2023年5—12月广州医科大学附属第一医院内镜中心消毒后软式内镜的活检腔道和吸引腔道进行采样。内镜储存的镜柜有3个不同的品牌。在内镜储存时,严格控制内镜存储的温湿度,使其保持在温度15~25℃、湿度41%~60%。采样时间对应内镜储存时间,分别在储存后的1、2、3、5、7 d进行采样。每个时间点计划采样的样本数为30条,纤支镜15条,消化内镜15条。2023年5—9月,在落实采样方案期间,已采样的145条内镜中,有1条储存3 d的内镜监测结果超标,结果为24 CFU/件,其余内镜都合格。因此,针对储存2 d内的内镜增加了采样数量,进一步通过监测结果来评估储存1~2 d的内镜安全性。结果 2023年5—12月共334条次内镜纳入监测,纤支镜纳入检测共185条次,消化内镜纳入检测149条次。在334条次内镜中,其中2条次内镜储存1、3 d的管腔菌落数分别为25 CFU/件、24 CFU/件,菌落数超标,其余332条次内镜监测结果都达标,总体合格率为99。4%。结论 在保证软式内镜高水平消毒效果的前提下,严格控制镜柜的温湿度,同时使用具备循环洁净空气干燥或紫外线消毒保证良好储存环境镜柜的情况下,非手术治疗的软式内镜高水平消毒后,储存2 d内的软式内镜是安全的,使用当天无需再处理。
Practical Study on Storage Period Safety of Soft Endoscope After High-Level Disinfection
Objective To investigate the safe storage time of soft endoscopes by monitoring the microbial contamination of the lumen of soft endoscopes stored in the mirror cabinet for different times after high-level disinfection. Methods Random sampling was used to select the biopsy and suction cavities of the flexible endoscopes after disinfection in the endoscopy center,the First Affiliated Hospital of Guangzhou Medical University from May to December 2023. There were three different brands of mirror cabinets for storing endoscopes. During endoscopic storage,the temperature and humidity of endoscopic storage should be strictly controlled to maintain a temperature of 15-25 ℃ and a humidity of 41%-60%. The sampling time corresponds to the endoscopic storage time,and samples were taken at 1,2,3,5 and 7 d after storage. At each time point,30 samples were planned to be sampled,15 samples were collected by fiberoptic bronchoscopy and 15 samples were collected by digestive endoscope. During the period from May to September 2023,among the 145 mirrors sampled,one of the endoscopic monitoring results stored for 3 d exceeded the standard,the result was 24 CFU/piece,and the rest of the endoscopes were qualified. Therefore,the number of samples for endoscopes stored within two days was increased,and the endoscopic safety of storage for 1-2 d is further evaluated by monitoring results. Results From May to December 2023,a total of 334 endoscopes were included in the monitoring,185 in fiberoptic bronchoscopy and 149 in digestive endoscopy. Among the 334 endoscopes,the colonies of two endoscopes stored for 1 and 3 d were 25 CFU/piece and 24 CFU/piece respectively,which exceeded the standard. The results of the other 332 endoscopes were qualified,and the overall qualified rate was 99.4%. Conclusion Under the premise of ensuring the high-level disinfection effect of the soft endoscope,strictly control the temperature and humidity of the mirror cabinet,and use the mirror cabinet with circulating clean air,dry or ultraviolet disinfection to ensure a good storage environment,after high-level disinfection of non-operative soft endoscope,the soft endoscope stored within 2 d is safe and does not need to be processed on the same day of use.
