Correlation Analysis Between Fibrinogen and D-Dimer Levels and Disease Activity in Gouty Arthritis
Objective To detect the levels of plasma fibrinogen (FIB) and D-dimer (D-D) in patients with gouty arthritis,and to explore their relationship with disease activity of gouty arthritis.Methods One hundred patients with gouty arthritis and 76 patients with ankylosing spondylitis (AS) who were hospitalized in the Department of Rheumatology and Immunology of the First Affiliated Hospital of Xiamen University,School of Medicine,Xiamen University from August 2022 to August 2023 were selected.All subjects were divided into gout group and AS group according to clinical diagnosis,and each group was subdivided into two groups according to whether the level of D-D and FIB was higher than the normal range.The clinical and laboratory indicators of all study subjects were recorded,FIB and D-D,and study their relationships with indicators such as erythrocyte sedimentation rate (ESR) and C reactive protein (CRP) in patients with gouty arthritis were detected.Results The D-D level in the gout group was (1.63±0.67) μg/mL,which was higher than that in the AS group[(0.64±0.11) g/mL],and the difference was statistically significant (P<0.001).Among gout patients,the erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) in the group with elevated D-D were 59.00 (32.75,86.00) mm/h and 19.00 (12.00,34.50) mg/L respectively,both of which were higher than those in the group with normal D-D[33.90 (6.65,108.00) mm/h and 5.70 (1.40,27.82) mg/L respectively],and the differences were statistically significant (P<0.001).The ESR and CRP in the group with elevated fibrinogen (FIB) were 59.00 (34.00,86.00) mm/h and 17.00 (9.50,30.50) mg/L respectively,both of which were higher than those in the group with normal FIB[36.00 (12.30,107.00) mm/h and 1.92 (0.97,5.78) mg/L respectively],and the differences were statistically significant (P<0.001).Correlation analysis showed that FIB and D-D were positively correlated with the ESR and CRP of gout patients respectively (r=0.797,0.645,P<0.001).Conclusion The levels of FIB and D-D in gout patients were higher than those in AS patients,and were positively correlated with disease activity.FIB and D-D could be used as reference indexes to evaluate disease activity in gouty arthritis.Aggressive anticoagulant therapy during active gouty arthritis may reduce the risk of cardiovascular or venous thrombosis in patients.
