Application Value and Characteristic Analysis of Surface Electrocardiogram in Left Posterior Septum of Preexcitation Syndrome
Objective To explore the application value and characteristics of surface electrocardiogram in the left posterior septum of preexcitation syndrome.Methods A total of 68 patients with preexcitation syndrome in Tianmen First People's Hospital from July 2019 to October 2023 were selected.According to the surgical diagnosis,all them were divided into left posterior septal atrioventricular accessory pathway (38 cases) and right posterior septal atrioventricular accessory pathway (30 cases).The morphological characteristics of atrial wave,P-R interval and electrocardiogram QRS complex of the two groups were analyzed.The surgical results were used as the "gold standard" to analyze the diagnostic value of surface electrocardiogram in the left posterior septum of preexcitation syndrome.Results In preexcitation syndrome,the direction of delta wave in V1δ,the Q/R ratio in P-R interval and transition zone in chest leads Ⅱ,Ⅲ and aVF were different from the morphology of QRS complex in V1,but the direction of delta wave in Ⅱ,Ⅲ and aVF was the same.In 38 patients with preexcitation syndrome,the left posterior septal ventricular rate of type A was higher than that of type B,the difference was statistically significant (P<0.05).The QRS duration was 0.05-0.10 s in 8 cases,accounting for 21.05%;19 cases of 0.11-0.29 s,accounting for 50.00%;The number of patients with ≥0.30 s was 11 cases,accounting for 28.94%,the difference between groups was statistically significant (P<0.05).The QRS complex had a variety of forms.There were 6 patients (18.75%) with delta QRS complex in all leads of electrocardiogram,and 32 patients (84.21%) with delta QRS complex in 5-9 leads of electrocardiogram,the difference between groups was statistically significant (P<0.05).The P-R spacing showed irregular characteristics,and the spacing time was 0.25-0.70 s.Receiver operating characteristic curve analysis showed that the area under curve value of the left posterior septum was higher than that of the right posterior septum in the diagnosis of preexcitation syndrome,the difference was statistically significant (P<0.05).Conclusion The surface electrocardiogram can accurately identify the left posterior septum of preexcitation syndrome and provide an important quantitative reference for clinical treatment.
