目的 分析音乐护理联合系统心理防御在干燥综合征患者生活质量改善中的作用。方法 选取2021年12月—2023年12月厦门大学附属第一医院风湿免疫科收治的86例干燥综合征患者为研究对象。根据不同护理方法分为研究组与对照组，各43例。对照组采用常规护理，研究组采用音乐护理联合系统心理防御干预。对比2组防御方式、心理弹性、生活质量、疾病疼痛、疾病活跃度及负性情绪。结果 干预后，研究组不成熟型防御机制评分、中间型防御机制评分分别为(3。25±0。34)分、(3。46±0。21)分，低于对照组的(4。38±0。51)分、(3。78±0。34)分，差异有统计学意义(P<0。05);研究组成熟性防御机制评分、掩饰度评分分别为(5。83±0。64)分、(6。97±0。42)分，高于对照组的(5。27±0。36)分、(6。45±0。51)分，差异有统计学意义(P<0。05)。研究组Connor-Davidson心理弹性量表(Connor-Davidson resilience scale，CD-RISC)评分及健康状况调查简表(the MOS item short from health survey，SF-36)评分分别为(80。63±10。25)分、(82。59±7。34)分，高于对照组的(69。47±12。33)分、(74。83±10。21)分，差异有统计学意义(P<0。05)。研究组干燥综合征疾病活动指数(Eurpoean League Against Rheumatism Sjögren syndrome disease activity index，ESSDAI)评分与干燥综合征患者报告指数(Eurpoean League Against Rheumatism Sjögren syndrome patient reported index，ESSPRI)评分分别为(2。34±1。02)分、(5。36±1。21)分，均低于对照组的(3。78±1。23)分、(7。45±2。33)分，差异有统计学意义(P<0。05)。研究组汉密尔顿焦虑量表(Hamilton anxiety scale，HAMA)评分、汉密尔顿抑郁量表(Hamilton depression scale，HAMD)评分分别为(8。36±1。29)分、(9。37±1。06)分，均低于对照组的(11。56±2。11)分、(12。53±2。41)分，差异有统计学意义(P<0。05)。结论 音乐护理联合系统心理防御有利于改善干燥综合征患者的生活质量与心理状况。
Influence of Music Nursing Combined With Systematic Psychological Defense on Quality of Life of Patients With Sj?gren Syndrome
Objective To analyze the effect of music nursing combined with systematic psychological defense on improving the quality of life of patients with Sjögren syndrome. Methods A total of 86 patients with Sjögren syndrome admitted to the department of rheumatology and immunology,the First Affiliated Hospital of Xiamen University,School of Medicine,Xiamen University from December 2021 to December 2023 were selected as the study objects. They were divided into control group and study group according to different nursing methods,43 cases in each group. The control group was treated with routine nursing,and the study group was treated with music nursing combined systematic psychological defense intervention. The defense mode,psychological resilience,quality of life,disease pain,disease activity and negative emotions of the two groups were compared. Results After the intervention,the immature defense mechanism score and intermediate defense mechanism score of the study group were (3.25±0.34) points and (3.46±0.21) points,which were lower than (4.38±0.51) points and (3.78±0.34) points of the control group,the differences were statistically significant (P<0.05). The maturity defense mechanism score and masking score of the study group were (5.83±0.64) points and (6.97±0.42) points,respectively,which were higher than (5.27±0.36) points and (6.45±0.51) points of the control group,the differences were statistically significant (P<0.05). The scores of Connor-Davidson resilience scale (CD-RISC) and the MOS item short from health survey (SF-36) of the study group were (80.63±10.25) points and (82.59±7.34) points,respectively,which were higher than (69.47±12.33) points and (74.83±10.21) points of the control group,the differences were statistically significant (P<0.05). The Eurpoean League Against Rheumatism Sjögren syndrome disease activity index (ESSDAI) score and the Eurpoean League Against Rheumatism Sjögren syndrome patient reported index (ESSPRI) scores of the study group were (2.34±1.02) points,(5.36±1.21) points,respectively,which were lower than (3.78±1.23) points,(7.45±2.33) points of the control group,the differences were statistically significant (P<0.05). The Hamilton anxiety scale (HAMA) and Hamilton depression scale (HAMD) scores of the study group were (8.36±1.29) points and (9.37±1.06) points,respectively,which were lower than (11.56±2.11) points,and (12.53±2.41) points of the control group,the differences were statistically significant (P<0.05). Conclusion Music nursing combined with systematic psychological defense is conducive to improving the quality of life and psychological status of patients with Sjögren syndrome.
