低温等离子射频消融术联合不同温度盐水鼻腔冲洗在腺样体肥大患儿治疗中的应用效果

目的 探讨低温等离子射频消融术联合不同温度盐水鼻腔冲洗在腺样体肥大患儿治疗中的应用效果。方法 选取 2021年 6 月—2023 年 9 月泉州医学高等专科学校附属人民医院收治的 80 例腺样体肥大患儿为研究对象,根据随机数字表法分为对照组与观察组,各 40 例。对照组行低温等离子射频消融术联合常温盐水鼻腔冲洗治疗,观察组则采用低温等离子射频消融术联合低温盐水鼻腔冲洗治疗。比较 2 组手术时间、术中出血量、住院时间、并发症发生率、术后不同时间的不适感程度[面部疼痛表情量表(faces pain scale-revised,FPS-R)评分]及水肿程度、手术前后的症状体征(鼻塞、打鼾及张口呼吸)积分及鼻腔容积。结果 观察组手术时间及住院时间分别为(13。96±3。03)min及(4。30±1。63)d,短于对照组的(16。25±3。56)min及(5。19±1。75)d,观察组术中出血量为(10。62±1。92)mL,少于对照组的(15。59±2。35)mL,差异有统计学意义(P<0。05)。观察组并发症总发生率为0,低于对照组的 17。50%,差异有统计学意义(P<0。05)。观察组术后 1、2、3 d FPS-R评分分别为(5。39±0。96)分、(3。96±0。69)分、(3。26±0。56)分,低于对照组的(6。10±1。03)分、(4。39±0。75)分、(3。62±0。59)分,差异有统计学意义(P<0。05)。观察组术后 2、3 d水肿程度优于对照组,差异有统计学意义(P<0。05)。术后 3、7 d,观察组鼻塞、打鼾、张口呼吸症状体征积分低于对照组,差异有统计学意义(P<0。05)。术后 3 个月,观察组鼻腔容积大于对照组,差异有统计学意义(P<0。05)。结论 低温等离子射频消融术联合低温盐水鼻腔冲洗在腺样体肥大患儿治疗中的应用效果相对较好,安全性更高,可有效控制手术创伤及不适感,因此在腺样体肥大患儿中的应用价值相对更高。
Application Effect on Low-Temperature Plasma Radiofrequency Ablation Combined With Nasal Irrigation With Saline at Different Temperatures in the Treatment of Children With Adenoid Hypertrophy
Objective To investigate the application effect of low-temperature plasma radiofrequency ablation combined with nasal irrigation with saline at different temperatures in the treatment of children with adenoid hypertrophy.Methods A total of 80 children with adenoid hypertrophy in People's Hospital Affiliated of Quanzhou Medical College from June 2021 to September 2023 were selected as the study objects,they were averagely divided into control group and observation group,and each group respectively included 40 cases.The control group were treated with low-temperature plasma radiofrequency ablation combined with nasal irrigation with saline at normal temperature,the observation group were treated with low-temperature plasma radiofrequency ablation combined with nasal irrigation with saline at low temperature.Then the operation time,intraoperative blood loss,hospitalization time,complications rates,degree of discomfort[faces pain scale-revised(FPS-R)score]and edema degree at different time after the operation,symptoms and signs(nasal congestion,snore and open mouth breathing)score and nasal volume before and after the operation of two groups were compared.Results The operation time and hospitalization time of the observation group were(13.96±3.03)min and(4.30±1.63)days,respectively,which were shorter than(16.25±3.56)min and(5.19±1.75)days of the control group,the intraoperative blood loss of the observation group was(10.62±1.92)mL,which was less than(15.59±2.35)mL of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications of the observation group was 0,which was lower than 17.50%of the control group,the difference was statistically significant(P<0.05).The FPS-R scores at postoperative 1,2 and 3 days of the observation group were(5.39±0.96)points,(3.96±0.69)points and(3.26±0.56)points,which were lower than(6.10±1.03)points,(4.39±0.75)points and(3.62±0.59)points of the control group,the differences were statistically significant(P<0.05).The degree of edema at postoperative 2 and 3 days of the observation group was better than that of the control group,and the differences were statistically significant(P<0.05).At postoperative 3 and 7 days,the symptom and sign scores of nasal congestion,snore and open mouth breathing of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).At postoperative 3 months,the nasal volume of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application effect of low-temperature plasma radiofrequency ablation combined with nasal irrigation with saline at low temperature in children with adenoid hypertrophy is relatively better,its safety is higher,and it can effectively control the surgical trauma and discomfort,so its application value in the children with adenoid hypertrophy is relatively higher.
