水蛭粉联合体外培育牛黄治疗急性缺血性中风的临床研究

目的 探讨水蛭粉联合体外培育牛黄治疗急性缺血性中风的临床效果。方法 选取阳江市中医医院脑病科 2023 年 1月—2024 年 6 月收治的 66 例急性缺血性中风住院患者,随机分为观察组与治疗组,各 33 例。观察组采用常规疗法,治疗组在观察组基础上采用水蛭粉联合体外培育牛黄治疗。比较 2组治疗前、治疗 7、14 d患者神经功能[美国国立卫生研究院卒中量表评分(National Institutes of Health Stroke scale score,NIHSS)]、临床症状、日常生活能力[巴氏量表(Barthel index,BI)指数]的水平;评价 2 组治疗前、治疗 14 d实验室指标[超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)]、血浆蛋白C(plasma protein C,PC)活性、抗凝血酶Ⅲ(antithrombin Ⅲ,AT-Ⅲ)活性]的变化;观察水蛭粉联合体外培育牛黄对急性缺血性中风的治疗作用;观测用药期间不良反应发生率。结果 治疗 7、14 d,治疗组NIHSS评分分别为(7。29±1。03)分、(4。71±0。79)分,低于观察组的(10。44±1。13)分、(7。36±0。95)分;治疗组临床症状评分分别为(18。56±3。55)分、(14。09±3。11)分,均低于观察组的(22。78±3。76)分、(18。14±3。23)分;而治疗组日常生活能力评分分别为(64。28±6。85)分、(75。69±7。28)分,高于观察组的(55。83±7。29)分、(67。44±8。44)分,差异均有统计学意义(P<0。05)。治疗14 d,治疗组hs-CRP水平为(7。29±1。86)mg/L,低于观察组的(13。77±2。41)mg/L;治疗组PC活性、AT-Ⅲ活性分别为(98。44±5。71)%、(99。27±2。23)%,高于观察组的(92。59±6。62)%、(95。44±2。06)%,差异均有统计学意义(P<0。05)。治疗组总有效率为 93。94%,高于观察组的75。76%,差异有统计学意义(P<0。05)。2 组均未发生严重不良反应。结论 水蛭粉联合体外培育牛黄治疗急性缺血性中风临床效果显著,可有效改善患者神经功能、临床症状与日常生活能力,其作用机制可能与下调炎症因子水平与提升PC活性、AT-Ⅲ活性相关。
Clinical Study on the Treatment of Acute Ischemic Stroke With Hirudo Powder Combined With in Vitro Cultured Calculus Bovis
Objective To explore the clinical effect of hirudo powder combined with in vitro cultured calculus bovis in the treatment of acute ischemic stroke.Methods A total of 66 inpatients with acute ischemic stroke admitted to the department of encephalopathy,Yangjiang Hospital of Traditional Chinese Medicine from January 2023 to June 2024 were eligible and were randomly divided into an observation group and a treatment group,with 33 cases in each group.The observation group received conventional therapy,while the treatment group was treated with hirudo powder combined with in vitro cultured calculus bovis on the basis of the observation group.The neurological function[(National Institutes of Health Stroke scale score(NIHSS)],clinical symptoms,and activities of daily living[(Barthel index(BI)]were compared before treatment,7 and 14 days after treatment between the two groups.The laboratory indicators[high-sensitivity C-reactive protein(hs-CRP),plasma protein C(PC)activity,antithrombin Ⅲ(AT-Ⅲ)activity]before treatment and 14 days after treatment were evaluated.And the therapeutic effect of hirudo powder combined with in vitro cultured calculus bovis on acute ischemic stroke and the incidence of adverse reactions during the medication period were assessed.Results After 7 and 14 days of treatment,the NIHSS scores of the treatment group were(7.29±1.03)points and(4.71±0.79)points,which were lower than(10.44±1.13)points and(7.36±0.95)points of the observation group,respectively,the clinical symptom scores of the treatment group were(18.56±3.55)points and(14.09±3.11)points,which were lower than(22.78±3.76)points and(18.14±3.23)points of the observation group,respectively.the scores of daily living ability of the treatment group were(64.28±6.85)points and(75.69±7.28)points,respectively,which were higher than(55.83±7.29)points and(67.44±8.44)points of the observation group,and the differences were statistically significant(P<0.05).After 14 days of treatment,the hs-CRP level of the treatment group was(7.29±1.86)mg/L,which was lower than(13.77±2.41)mg/L of the observation group,the activity of PC and AT-Ⅲ of the treatment group were(98.44±5.71)%and(99.27±2.23)%,respectively,which were higher than(92.59±6.62)%and(95.44±2.06)%of the observation group,the differences were statistically significant(P<0.05).The total effective rate of the treatment group was 93.94%,which was higher than 75.76%of the observation group,and the difference was statistically significant(P<0.05).No serious adverse reactions occurred in both groups.Conclusion The treatment of acute ischemic stroke with hirudo powder combined with in vitro cultured calculus bovis has significant clinical effects,which can effectively improve the neurological function,clinical symptoms,and daily living ability of patients,and its mechanism of action may be related to the down-regulation of inflammatory factors and the enhancement of PC activity and AT-Ⅲ activity.
