药学类医疗服务价格政策回顾、项目设立与启示

摘要
药学服务对促进合理用药、提高医疗质量、保证患者用药安全具有重要意义.文章在界定临床药师提供药学类医疗服务范围、梳理国家层面药学医疗服务价格项目设立现状的基础上,回顾了典型地区从试点设立"药事服务费"项目或修订现有项目阶段,到作为按现有价格项目子项目兼容方式加收阶段的转变历程,以药学门诊诊查费、住院诊查费(临床药学加收)、静脉药物配置费为例,比较了样本省份药学类价格项目要素和收费水平.典型地区做法给其他省市规范药学类价格项目管理以下启示:以临床价值和独立服务产出为导向,合理新增药学类价格项目;明确不同应用场景加收标准,合理修订兼容药学类价格项目.
Abstract
Pharmaceutical care plays an important role in promoting rational drug use,improving medical quality and ensuring drug safety for patients.On the basis of defining the scope of pharmaceutical medical services provided by clinical pharmacists and sorting out the status quo of the establishment of pharmaceutical medical service price items at the national level,it reviewed the transformation process of typical areas from the pilot establishment of"pharmaceutical service fee"items or the revision of existing items to the stage of adding fees in a compatible way according to the existing price items.Taking pharmacy outpatient examination fee,inpatient examination fee(clinical pharmacy plus charge)and intravenous drug dispensing fee as examples,the factors and charging levels of pharmaceutical price items in sample provinces were compared.The typical regional practice has the following implications for other provinces and cities to standardize the pharmaceutical price project management:rationally adding new pharmacy price items based on clinical value and independent service output;clarifying the criteria for charging additional fees in different application scenarios;and rationally revising compatible pharmacy price items.
