2019年—2022年丽水市非结核分枝杆菌菌种分布研究

摘要
目的 分析丽水市非结核分枝杆菌(NTM)分布情况,为科学制订丽水市NTM病防控策略提供依据.方法 采用基因芯片法对丽水市2019年—2022年结核分枝杆菌抗原检测为阴性的液体培阳菌株行菌种鉴定,对2019年—2022年度NTM分离率、菌种分布进行分析.结果 2019年—2022年NTM检出率稳定,各年度间差异无统计学意义(x2趋势=0.765,P=0.382);分离出常见的11个菌种,占前3位的分别为胞内分枝杆菌(58.24%)、鸟分枝杆菌(17.65%)、龟/脓肿分枝杆菌(7.06%);不同性别NTM检出率差异有统计学意义(x2=23.004,P<0.001),其中男性NTM检出率(9.39%)低于女性NTM检出率(18.59%);不同年龄组间NTM检出率差异有统计学意义(x2=47.698,P<0.001),NTM感染者主要为51岁以上的中老年人(88.82%).结论 2019年—2022年丽水市NTM分离率稳定,菌种分布居于前三的为胞内、鸟、龟/脓肿分枝杆菌,感染人群以女性、中老年人为主.
Abstract
Objective This paper aims to analyze the distribution of non-tuberculous Mycobacterium(NTM)in Lishui City,so as to provide basis for scientific prevention and control strategy of NTM disease in Lishui City.Methods The liquid-cul-tured positive strains that were negative in Mycobacterium tuberculosis antigen test in Lishui City from 2019 to 2022 were identi-fied by gene microarray method.The isolation rate and the distribution of NTM from 2019 to 2022 were analyzed.Results The detection rate of NTM was stable from 2019 to 2022,and there was no statistical significance on the difference between different years(x2tendency=0.765,P=0.382).Eleven common strains were isolated,and the top three were Mycobacterium intracellular(58.24%),Mycobacterium avium(17.65%),and Mycobacterium turtle/abscess(7.06%).There was statistical significance on the detection rate of NTM between genders(x2=23.004,P<0.001),and the detection rate of NTM in males(9.39%)was lower than that in females(18.59%).There was statistical significance on the differences in the detection rate of NTM among different age groups(x2=47.698,P<0.001),and the infected patients were mainly the middle-aged and elderly peo-ple over 51 years old(88.82%).Conclusion The isolation rate of NTM in Lishui City was stable from 2019 to 2022.The top three strains are Mycobacterium intracellulare,Mycobacterium avium,Mycobacterium chelonis/abscess,and the infected popula-tion is mainly female,middle-aged and elderly.
