Relationship between glucolipid metabolism, sex hormones and gut microbiota disturbance in patients with polycystic ovary syndrome
Objective To observe the changes of glucolipid metabolism and sex hormones, and analyze their relationship with gut microbiota disturbance(GMD)in patients with polycystic ovary syndrome(PCOS).Methods A total of 132 pa-tients with PCOS admitted to the hospital and 132 healthy controls between June 2020 and June 2023 were enrolled as the PCOS group and control group. The changes of glucolipid metabolism indices and sex hormones indices were compared between the two groups. The glucolipid metabolism indices includes total cholesterol(TC),triglyceride(TG),fasting in-sulin(FINS),insulin resistance index(HOMA-IR),fasting blood glucose(FBG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),and the sex hormones indices included estradiol(E2),testosterone(T),luteinizing hormone(LH)and follicle-stimulating hormone(FSH).According to the severity of GMD,the patients in the PCOS group were divided into the degree Ⅰ group(n=39),the degree Ⅱ group(n=51)and the degree Ⅲ group(n=42);the changes of glucolipid metabolism and sex hormones were compared among the three groups. The relationship of the sever-ity of GMD with glucolipid metabolism and sex hormones was analyzed using Spearman correlation analysis. Results The levels of TC,TG,LDL-C,FINS,HOMA-IR,E2,T and LH in the PCOS group were higher than those in the control group(P<0.001).There were no significant differences in HDL-C,FBG or FSH between the two groups(P>0.05).The levels of TC,TG,LDL-C,FINS and HOMA-IR in the degree Ⅰ group were lower than those in the de-gree Ⅱ group and degree Ⅲ group(P<0.05),and those in the degree Ⅲ group were higher than in the degree Ⅱ group(P<0.05),respectively.There was no significant difference in HDL-C(F=62.730,P=0.208)or FBG(F=0.101,P=0.904)among the three groups. The levels of E2,T and LH in the degree Ⅰ group were lower than those in the degree Ⅱ group and degree Ⅲ group(P<0.05),and those in the degree Ⅲ group were higher than in the degree Ⅱ group(P<0.05),respectively.There was no significant difference in FSH among the three groups(F=0.405,P=0.668).Spearman correlation analysis showed that TC,TG,LDL-C,HOMA-IR,FINS,T and LH were all positively correlated with the severity of GMD(P<0.05).Conclusion There are glucolipid metabolism disorders and abnormal changes of sex hormones in PCOS pa-tients,and they may be mutual influencing factors of gut microbiota disturbance.
