先兆流产患者血清HSP70和PD-L1水平及其与阴道微生态指标的相关性

目的 分析先兆流产患者血清热休克蛋白70(HSP70)、程序性死亡受体-配体l(PD-L1)水平及其与阴道微生态指标的相关性,为该类患者的治疗提供参考。方法 选取我院2021年10月至2022年10月收治的98例先兆流产患者作为病例组,选择同期进行体检的健康妊娠期妇女100例为健康组。采用酶联免疫吸附法检测血清HSP70、PD-L1水平;采用高通量测序技术分析妊娠期妇女阴道菌群物种分布和丰度;采用Pearson相关性分析血清HSP70、PD-L1水平及其与阴道微生态指标和炎症因子的相关性。结果 病例组患者血清HSP70、PD-L1水平显著低于健康组(均P<0。05)。病例组患者肠道放线菌门、变形菌门、拟杆菌门相对丰度显著高于健康组,梭杆菌门相对丰度显著低于健康组(均P<0。05)。病例组患者肠道加德纳菌属和普氏菌属相对丰度显著高于健康组(P<0。05),而两组妊娠期妇女肠道链球菌属、梭杆菌属相对丰度比较差异均无统计学意义(均P>0。05)。病例组与健康组对象阴道pH值比较差异有统计学意义(P<0。05);病例组患者TNF-α、IFN-γ、IL-2、IL-6水平显著高于健康组,而IL-4水平低于健康组(均P<0。05)。相关性分析结果表明,血清HSP70与PD-L1水平呈正相关(r=0。409,P<0。001),血清HSP70、PD-L1水平与肠道放线菌门、变形菌门、拟杆菌门、加德纳菌属、普氏菌属、TNF-α、IFN-γ、IL-2、IL-6水平均呈负相关,与梭杆菌门、IL-4水平均呈正相关(均P<0。05)。结论 先兆流产患者血清HSP70、PD-L1水平降低,与阴道微生态指标密切相关。
The levels of serum HSP70 and PD-L1 in patients with threatened abortion and their correlation with vaginal microecological indicators
Objective To analyze the serum levels of heat shock protein 70(HSP70)and programmed death ligand 1(PD-L1)in patients with threatened abortion and their correlation with vaginal microecological indicators,providing a reference for the treatment.Methods From October 2021 to October 2022,98 patients with threatened abortion admitted to our hospital were included as the case group,and 100 healthy pregnant women who underwent physical examination were as the health group.Enzyme linked immunosorbent assay was applied to detect serum levels of HSP70 and PD-L1,high-throughput sequencing was applied to analyze the species and abundance of vaginal flora in pregnant women.Pearson method was applied to analyze the levels of serum HSP70 and PD-L1,and their correlation with vaginal microecological indicators and inflammatory factors.Results The serum levels of HSP70 and PD-L1 in case group were obviously lower than those in healthy group(all P<0.05).The relative abundances of Actinomycetota,Pseudomonadota and Bacteroidota in case group were obviously higher than those in health group,and the relative abundance of Fusobacteria was obviously lower than that in health group(all P<0.05).At the genus level,the relative abundances of Gardnerella and Pleurotus in case group were obviously higher than those in health group(all P<0.05),and those of Streptococcus and Fusobacterium in the two groups were not obviously different(all P>0.05).There was an obvious difference in vaginal pH between case group and healthy group(P<0.05).The levels of TNF-α,IFN-γ,IL-2 and IL-6 in case group were obviously higher,while that of IL-4 was lower than that in healthy group(all P<0.05).Correlation analysis showed that serum levels of HSP70 and PD-L1 were positively correlated(r=0.409,P<0.001),and both levels were negatively correlated with Actinomy-cetota,Pseudomonadota,Bacteroidota,Gardnerella,Prevotella,TNF-α,IFN-γ,IL-2,IL-6,while positively correlated with Fusobacteria and IL-4(all P<0.05).Conclusion The serum levels of HSP70 and PD-L1 in patients with threatened abortion decrease,and are closely related to vaginal microbiota indicators.
