摘要
目的 探讨清化益胃汤结合气交灸对脾虚气滞血瘀证慢性萎缩性胃炎(CAG)患者幽门螺杆菌(H.pylori)根除率及Musashi-1 mRNA表达的影响,为该类患者的治疗提供参考.方法 选取我院2022年1月至2023年10月收治的200例CAG患者为研究对象.入选患者按照随机对照原则分为4个组,均50例,其中对照组接受标准幽门螺杆菌四联疗法治疗,观察1组在对照组基础上予以清化益胃汤,观察2组在对照组基础上予以气交灸,观察3组在对照组基础上予以清化益胃汤联合气交灸.对比四组患者临床疗效、炎症因子水平、Musashi-1 mRNA表达水平、H.pylori清除率及生活质量.结果 治疗后,观察3组患者总有效率(92.00％)最高.治疗后四组患者白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均降低,血清胃泌素-17(G17)水平均升高;观察3组患者IL-6、TNF-α水平均低于对照组、观察1组和观察2组,G17水平高于对照组、观察1组和观察2组(均P＜0.05).治疗后四组患者Musashi-1 mRNA表达水平均降低,其中观察3组低于对照组、观察1组和观察2组(F=4.551,P＜0.05).治疗后观察3组患者H.pylori清除率(92.00％)高于对照组(74.00％)、观察 1 组(76.00％)和观察 2 组(78.00％)(x2=4.204,P＜0.05).治疗后,四组患者生活质量水平均提升,其中观察3组生活质量评分高于对照组、观察1组和观察2组高(均P＜0.05).结论 清化益胃汤结合气交灸能抑制脾虚气滞血瘀证慢性萎缩性胃炎患者Musashi-1 mRNA表达,提升H.pylori根除率和生活质量,降低炎症反应,疗效显著.
Abstract
Objective To observe the effects of Qinghua Yiwei Decoction combined with Qijiao moxibustion on Helico-bacter pylori(H.pylori)eradication rate and Musashi-1 mRNA expression in patients with chronic atrophic gastritis(CAG)of spleen deficiency Qi stagnation and blood stasis,providing a reference for the treatment.Methods A total of 200 pa-tients with CAG treated in our hospital from January 2022 to October 2023 were enrolled and randomly divided into 4 groups(n=50),receiving standard H.pylori quadruple therapy;additionally,based on this,the observation group 1 re-ceived Qinghua Yiwei Decoction,observation group 2 received Qijiao moxibustion,and observation group 3 received Qinghua Yiwei Decoction combined with Qijiao moxibustion.The clinical efficacy,inflammation level,Musashi-1 mRNA expression level,H.pylori eradication rate and quality of life were compared among the 4 groups.Results After treat-ment,the total efficiency rate in the observation group 3(92.00％)was the highest.In the 4 groups,the levels of interleukin-6(IL-6)and tumor necrosis factor α(TNF-α)decreased,and serum gastrin-17(G17)level increased.The levels of IL-6 and TNF-α in observation group 3 were lower,while G17 level was higher than in the control group,observation group 1 and observation group 2,respectively(all P＜0.05).After treatment,Musashi-1 mRNA expression level decreased in all groups,while that in the observation group 3 was lower than in the control group,observation group 1 and observation group 2,respectively(F=4.551,P＜0.05).H.pylori eradication rate in observation group 3(92.00％)was higher than in control group(74.00％),observation group 1(76.00％),and observation group 2(78.00％)resepctively(x2=4.204,P＜0.05).After treatment,the quality of life in the four groups was improved,and that in the observation group 3 was high-er than in the control group,observation group 1 and observation group 2,respectively(all P＜0.05).Conclusion Qinghua Yiwei Decoction combined with Qijiao moxibustion can inhibit the expression of Musashi-1 mRNA in chronic atrophic gastritis patients with spleen deficiency Qi stagnation and blood stasis syndrome,improve the eradication rate of H.pylori and quality of life,and reduce the inflammatory response,with significant effect.
