The role of Baduanjin training in improving type 2 diabetes based on gut microbiota,serum inflammatory factors,and intestinal mucosal barrier function
Objective To explore the regulatory effect of Baduanjin training on blood glucose homeostasis,serum in-flammatory factors,intestinal flora and intestinal mucosal barrier function in patients with type 2 diabetes(T2DM).Method Thirty patients with T2DM underwent a 6-month Baduanjin training.The body composition was detected using bioelectrical impedance analysis.Biochemical indicators such as fasting insulin,fasting blood glucose,total cholesterol and triglycerides were detected with automatic biochemical analyzer.Glycated hemoglobin(HbA1c)was detected with fully automated glycated hemoglobin analyzer.Enzyme linked immunosorbent assay was used to detect the levels of serum TNF-α,IL-6,IL-1β,IL-10,C-reactive protein(CRP),D-lactate(D-LA),Zonulin and milk fat globule epidermal growth factor-8(MFG-E8).HOMA-IR was analyzed.16S rRNA sequencing technology was used to analyze the gut microbiota of fecal samples.Results The six months of Baduanjin training significantly reduced the weight,BMI,waist circumference and body fat percentage of T2DM patients,and significantly increased lean body mass(all P<0.05).Fasting blood glucose and levels of HbAlc,HOMA-IR,CRP,TNF-α,IL-6,IL-1β,IL-8 and total cholesterol significantly reduced,while that of IL-10 significantly increased(all P<0.05).The intestinal flora Chaol index and Shannon index significantly increased;the abundances of Lactobacillus and Bifidobacterium significantly increased,while those of Escherichia coli,Enterococcus,Fungi and Candida albicans significantly decreased(all P<0.05).The levels of D-LA and Zonulin significantly decreased,while that of MFG-E8 increased(all P<0.05).The abundance and diversity of gut microbiota in T2DM patients signific-antly decreased,the abundance of beneficial bacteria significantly decreased,intestinal permeability increased,and system-ic inflammation levels elevated.Conclusion Baduanjin exercise can improve the blood glucose homeostasis,the richness and diversity of gut microbiota,intestinal mucosal barrier function,and systemic inflammatory status in T2DM patients.Improving gut microbiota may be an important step in personalized treatment for T2DM patients.
