Objective To construct a regional integrated pharmaceutical service mode of chronic disease hierarchical manage-ment.Methods A quality control circle group was established and based on the current level analysis,the key points such as team,information and process were clarified,and improvement was made from the aspects of building an information platform,establishing a hierarchical management system,and building a science popularization education system.Results A whole-process regional phar-maceutical cooperation chronic disease hierarchical management service mode of tertiary general hospital,community health service cen-ter and patient family was established,which improved the disease control compliance rate and drug treatment satisfaction rate of chronic disease patients.Conclusion Establishing a reliable management mode,fully mobilizing resources from regional central hospitals,communities and households,improving pharmaceutical service process,can improve the health management level and quality of life of chronic disease patients.
