上海市级医院四级日间手术运行情况分析

摘要
目的 了解上海市级医院四级日间手术运行情况.方法 基于上海市级医院医疗质量评价与促进平台,获取上海市级医院2022年-2023年四级日间手术相关数据,对开展例数、开展病种、占比情况、相关性指标等进行分析.结果 上海市级医院2022年共开展四级日间手术6 168例,涉及898个术种;2023年共开展四级日间手术54 563例,涉及3 066个术种.费用性指标方面,2023年四级日间手术的经济性水平较2022年有所改善;效率性指标方面,2022年-2023年四级日间手术相关指标变化不大,基本上没有统计学意义;安全性指标方面,院内新发肺部感染率、手术患者并发症发生率、手术切口感染率均为零,0～7 d重返率、0～14 d重返率均有所下降,但住院患者抗菌药物使用率升高,且差异有统计学意义(P<0.05).结论 上海市级医院四级日间手术的手术例数和手术术种有所增加,相关性指标有所改善.在保障医疗质量与安全的前提下,尚需通过规范化、精细化、同质化的管理,促进四级日间手术持续改进.
Abstract
Objective To investigate the operational status of grade Ⅳ day surgery in municipal hospitals in Shang-hai.Methods Based on the Medical Quality Evaluation and Promotion Platform for municipal hospitals in Shanghai,relevant data on grade Ⅳ day surgery from 2022 to 2023 in municipal hospitals in Shanghai were obtained.The number of cases performed,the types of diseases treated,the proportion of cases,and relevant indicators were analyzed.Re-sults In 2022,a total of 6 168 cases of grade Ⅳ day surgery involving 898 surgery types were conducted in municipal hospitals in Shanghai,while in 2023,a total of 54 563 cases involving 3 066 surgery types were performed.In terms of cost-related indicators,the economic efficiency of grade Ⅳ day surgery improved in 2023 compared to 2022.In terms of efficiency-related indicators,there was little change in the relevant indicators of grade Ⅳ day surgery from 2022 to 2023,with no statistically significant differences observed.In terms of safety indicators,the incidence rates of new-on-set pulmonary infections within the hospital,surgical complications,and surgical site infections were all zero.The read-mission rates within 0 to 7 days and 0 to 14 days both decreased,but the utilization rate of antibacterial agents among in-patients increased,with statistical significance(P<0.05).Conclusion The number of cases and the types of surger-ies treated in grade Ⅳ day surgery in municipal hospitals in Shanghai have increased,with improvements in relevant indi-cators.While ensuring medical quality and safety,it is necessary to promote continuous improvement of grade Ⅳ day surgery through standardized,refined,and homogenized management.
