The investigation of the application of vacuum sealing drainage (VSD) dressing in the treatment of extensive skin avulsion of the limb
Objective To investigate the use of vacuum sealing drainage (VSD) dressing in extensive skin avulsion in clinical settings. Methods Over the period of January 2009 to June 2011 in our hospital, 16 cases of extensive skin avulsion of the limb was treated by keeping the flap with adequate blood supply, and removing the flap that has no blood supply and seriously avulsed. The Padgett dermatome was used to cut the avulsed flap into intermediate split thickness skin graft, holes were poked all over the surface of the skin graft. The skin graft was placed on the wound and covered with Vacuum Sealing Drainage dressing. Then by continuous negative pressure suction, the negative pressure was adjusted to - 300~-450 mm Hg. After 5 to 7 days, the VSD dressing was removed and the survival of skin graft was observed,high pressure was then applied to the wound using bandage for 7 days before the stitches were removed. Results The wounds recovered satisfactorily after continuous negative pressure suction. In 10 cases, all of the akin was survived, the wound healed completely. In 6 cases, the skin had some necrosis, the wound healed after changing the gauze dressing. In the follow-up for 3-6 months, the skin that was grafted on the wound survived well, the patients had mild scar hyperplasia beside the periphery, and the limb function recovered satisfactorily. Conclusion The use of vacuum sealing drainage dressing is a simple and effective method in the treatment of extensive skin avulsion of the limb, and this method oi therapy is proved to be efficient in clinical application.
