摘要

目的 探讨液基薄层细胞学检测(TCT)技术在妇科子宫颈病变细胞学筛查中的临床应用价值.方法 选择2006-12-2010-12来包头医学院第一附属医院妇科就诊并接受宫颈病变筛查的患者8 492例,采用TCT技术检测及TBS分类标准诊断,对细胞学检查阳性者行阴道镜指引下多点活检,将细胞学检查结果与组织活检结果作比对分析.结果 8 492例TCT细胞涂片中,8 465例为满意标本,细胞学阳性结果为430例,阳性检出率5.1%,其中351例经活检组织病理诊断,CIN及以上病例共243例,TCT的敏感度为69.2% ;90例LSIL、58例HSIL及21例宫颈癌的细胞学检查与组织病理活检符合率分别为72.2%(65/90)、96.5%(56/58)、85.7%(18/21).结论 TCT检查能较为准确的反映宫颈病变情况,在妇科宫颈病变筛查中具有一定的临床应用价值,可作为临床宫颈癌及宫颈癌前病变的普查方法.

Abstract

Objective To explore the clinical application value in cervical lesions with gynecologic cytology screening, using thinprep cytology test. Methods Select 8492 patients who accept screening (or cervical lesions in our hospital from December 2006 to December 2010, adopt TCT detection and TBS Detection of diagnostic classification criteria. The cytology-positive patients take colposcopy biopsy under the guidance of multi-point, and then doing comparative analysis with cytology results and biopsy results. Results The 8465 cases were satisfied in 8492 cases , the positive detection rate was 5.1% in 430 cytology positive cases of 8 465, 243 cases were CIN of 351 cases with biopsy, final cincidence rate between cervix Of uterus cytology and histopathologic was 69.2%. The coincidence rates of TCT and pathologic histology were 72. 2% (65/90) in LS1L,96. 5%(56/58) in HSIL and 85.1% (18/21)in cervical carcinoma respectively. Conclusions TCT checking can exactly reflect cervical lesions. It can be used to check Cervical cancer with its high clinical application value in screening gynecological cervical lesions.
