Stat3、HIF-1α、MMP-7和VEGF在肾母细胞瘤中的表达

目的 研究血管内皮生长因子(vascular endothelial growth factor,VEGF),信号传导与转录激活因子3(signal transduccer and activator of transcription 3,Stat 3)、低氧诱导因子HIF-1α(hypoxia-inducible factor-1α,HIF-1α)和基质金属蛋白酶-7(MMP-7)在肾母细胞瘤(nephrohlastoma,Wilms'tumor,WT)中的表达及其及其与临床分期和病理分级间的关系.方法 采用免疫组化SP法检测43例手术切除WT标本和24例小儿非肿瘤肾组织标本中Stat 3、HIF-1α、MMP-7、与VEGF的表达.结果 24例对照组织标本中MMP-7未见表达,43例WT组织标本中MMP-7在胞浆表达率54.18±10.72(MVD值).MMP-7在预后良好(favorable histological types,FH)组中的阳性表达率明显低于在预后不良(unfavorable histological types,UH)组中的阳性表达率(P<0.05); VEGF、Star 3及HIF-1α在WT组织中表达强度较正常肾组织显著增强(P<0.05).另外,临床分期Ⅲ~Ⅳ和预后不良病理类型的WT中Star 3及VEGF表达强度明显高于临床分期Ⅰ~Ⅱ的WT,预后不良病理类型及直径≥6 cm的WT中HIF-1α表达强度较预后良好型及直径<6cm的WT升高.VEGF的表达与性别、年龄和病理学分型无关(P>0.05),而与临床分期明显相关(P<0.05).结论 WT组级中Stat 3、HIF-1α、MMP-7和VEGF的表达与肿瘤侵袭转移和发展有关,可被认为是肾母细胞侵袭性标志,对WT对于靶向治疗肾母细胞瘤具有一定意义.
Stat3, HIF-1α, MMP-7 and VEGF in Wilms tumor expression and clinical significance
Objective Detection of Wilms tumor (nephrohlastoma, wilms tumor, WT) tissues of Stat3, HIF-lα, MMP-7and VEGF expression and clinical stage and pathological grade relationships. Methods lmmunohistochemical SP method detected 43 cases of surgical specimens and 24 eases of children with WT non-tumor kidney tissues Stat3, HIF-la,MMP-7and VEGF expression. Results 24 cases in control tissues did not express MMP-7, 43 cases of WT tissue samples MMP-7 expression in the cytoplasm of 54.18±10. 72(MVD). MMP-7 in a good prognosis (favorable histological types, FH) positive expression group was significantly lower than in the poor prognosis (unfavorable histological types, UH) group, positive expression rate (P<0. 001); The expression of Stat3, HIF-lo and VEGF were significantly up-regulated in Wilms'tumor tissues compared to those in adjacent tissues and normal kidney tissues (P<0. 05). Stat3 and VEGF proteins in Wilms'tumor tissues of Stage Ⅲ~Ⅳ were significantly higher than those of stage Ⅰ~Ⅱ The higher expression of HIF-1 in Wilms'tumor tissues was shown in tumors with high risk histopathology . VEGF in Wilms tumor tissues was significantly higher than children of non-tumor kidney tissue (P <0. 05). VEGF expression significantly correlated with the clinical stage (P<0.05). Conclusions WT tissue Stat3, H1F-1.MMP7 and VEGF expression and tumor invasion, metastasis and development, can be considered invasive signs of Wilms on WTs clinical stage and pathologic grade and prognosis significance.
