自我管理教育改善肺结核患者焦虑抑郁状态的效果研究

目的 探讨自我管理教育改善肺结核焦虑抑郁状态的有效性.方法 将294例患者分为观察组和对照组,对照组给予常规治疗和护理,观察组在此基础上实施自我管理教育.评价两组教育前后焦虑抑郁状态、自理行为、生存质量.结果 观察组焦虑自评量表(SAS)及抑郁自评量表(SDS)的评均明显低于教育前,差异具有统计学意义(P<0.01),与对照组比较,教育后观察组SAS、SDS症状评分明显低于对照组;差异具有统计学意义(P<0.01);观察组自我管理教育后自力行为的日常生活能力、药物自我处理、症状自我处理、心理自我调节、社会交往行为5个自我管理方面的评分比教育前明显提高,差异具有统计学意义(P<0.01),与对照组比较,观察组自理行为水平的5个自我管理方面的分明显高于对照组,差异具有统计学意义(P<0.01);生存质量健康调查简表(SF-36)观察组实施自我管理教育后生理功能、生理职能、躯体功能、总体健康、活力、社会功能、情感职能和精神健康8个维度评分显著提高,差异具有统计学意义(P<0.01),与对照组相比,观察组8个维度各维度得分均高于对照组,差异具有统计学意义(P<0.05).结论 自我教育管理改善肺结核焦虑和抑郁状态,同时在提高患者的自力行为和生存质量均有积极作用.
Study on effect of self-control education in improvement of anxious despondent condition of patient with pulmonary tuberculosis
Objective To discuss the validity of self-control education in improvement of anxious despondent condition of patients with pulmonary tuberculosis. Methods 294 example patients were Divided into the observation group and the control group. The control group gived the conventional treatment and nursing, the observation group implemented the self-education management in this foundation. Around appraises two groups to educate the anxious despondent condition,to take care of oneself the behavior, the survival quality. Results The observation group from comments anxious meter(SAS) and from comments meter(SDS) to comment despondently obviously was lower than before the education, the difference had statistics significance(P<0.01) .compared with the control group,after the education observation group SAS.SDS symptom grading was lower than the control group obviously; The difference had statistics significance) P<0.01); After the observation group self-control education self-reliant behavior daily life ability,medicine self-processing,symptom self-processing,the psychological self-adjustment,the society associate the behavior 5 self-control aspects grading to enhance obviously compared to the education before,the difference had statistics significance( P<0. 01),compared with the control group, the observation group took care of oneself the behavior level 5 self-control aspect minute to be higher than the control group obviously,the difference had statistics significance(P<0. 01); After the survival quality health investigation simple list(SF-36) observation group implemented the self-control education the physiological function, the physiological function, the body function, the overall health,the vigor,the social function, the emotion function and the energetic health 8 dimension grading remarkable enhancement,the difference had statistics significance(P<0. 01),compared with the control group,the observation group 8 dimension various dimensions score was higher than the control group, the difference had statistics significance (P<0. 05). Conclusion Self-education management improvement pulmonary tuberculosis anxious and despondent condition, simultaneously is enhancing patient's self-reliant behavior and the survival quality has the positive role.
