The Meta-analysis for prevention of ventilator-associated pneumonia by subglottic irrigation with chlorhexidine solution
Objective To evaluate the effect of subglottic irrigation with chlorhexidine solution on the prevention of ventilator-associated pneumonia(VAP)in patients with mechanical ventilation by Meta-analysis.Methods From the establishment of the database to November 2022,PubMed,Cochrane Library,Web of Science,Embase,CNKI,Vip,and Wanfang databases were retrievaled for the trials of patients with mechanical ventilation who used chlorhexidine irrigating solution for subglottic irrigation(test group)and other irrigating solutions for subglottic irrigation or no irrigation(control group)to prevent VAP.Meta analysis were conducted with RevMan 5.4.Results A total of 12 articles were included,including 1 191 patients with mechanical ventilation in ICU.Subglottic irrigation with chlorhexidine flushing solution reduced the secretions in the upper and lower respiratory tract of the balloon,and reduced the incidence of VAP,especially late VAP,but the effect on early VAP was not obvious(P=0.08).The operation could short the time of mechanical ventilation and ICU hospital stay of patients,but didn't reduce the rate of re-intubation and mortality.Conclusion Subglottic irrigation with chlorhexidine solution is recommended to prevent VAP.
