远端塌陷型颈内动脉假性闭塞腔内治疗的中远期疗效

摘要
目的 探讨颈动脉支架植入术(carotid artery stenting,CAS)应用于远端塌陷型颈内动脉假性闭塞患者的中远期疗效.方法 回顾性分析 2018 年 1 月至 2019 年 12 月复旦大学附属中山医院 43 例用CAS治疗的远端塌陷型颈内动脉假性闭塞患者临床资料,观察CAS的技术成功率、围手术期并发症发生率、5 年支架内再狭窄率和免于同侧脑梗/死亡/心梗率.结果 手术技术成功率 100%.围手术期 1 例患者出现高灌注综合征,并发症发生率 2.3%.患者术后随访 36.5~68.1 个月,中位随访时间为 63.0 个月,3 例发生支架内再狭窄,同侧脑梗死复发 5 例,死亡 2 例,无心肌梗死.随访 5 年免于支架内再狭窄率为 91.2%.5 年免于死亡/心梗/同侧脑梗率为 82.7%.结论 CAS应用于颈动脉远端塌陷型假性闭塞患者的技术成功率和中远期疗效均满意.
Abstract
Objective To investigate the mid-and long-term efficacy of carotid artery stenting(CAS)in the treatment of patients with carotid near occlusion with full collapse.Methods The clinical data of 43 patients with carotid near occlusion with full collapse who underwent CAS in Zhongshan Hospital,Fudan University from January 2018 to December 2019 were analyzed retrospectively.The technical success rate,perioperative complications of CAS,incidence,5-year in-stent restenosis rate,and freedom from death/myocardial infarction/stroke incidence rate were observed.Results The surgical technical success rate was 100%.One patient occurred hyperperfusion syndrome during the perioperative period,and the perioperative period complication rate was 2.3%.The patients were followed up for 36.5 to 68.1 months after surgery,with a median follow-up time of 63.0 months.In-stent restenosis occurred in 3 cases,recurrence of ipsilateral cerebral infarction in 5 cases,and death in 2 cases.There was no myocardial infarction.The 5-year follow-up rate of freedom from in-stent restenosis was 91.2%and the 5-year freedom from death/myocardial infarction/stroke incidence rate was 82.7%.Conclusion The technical success rate and mid-to long-term efficacy of CAS in patients with carotid near occlusion with full collapse are satisfactory.
