老年下肢深静脉血栓形成患者复发的影响因素分析

摘要
目的 分析老年患者下肢深静脉血栓形成(deep vein thrombosis,DVT)复发的影响因素.方法 连续性收集2017年1月至2018年9月山西白求恩医院住院治疗的500例首次确诊为下肢DVT的老年患者临床资料,采用Cox比例风险模型进行多因素分析确定患者DVT复发的影响因素.结果 85例患者复发DVT,复发率为17％.Cox多因素回归分析显示,男性[风险比(hazard ratio,HR)=1.701,95％置信区间(confidence level,CI)=1.094～2.644]、残余血栓(HR=3.033,95％CI=1.735～5.301)、静脉曲张(HR=1.715,95％CI=1.098～2.678)、蛋白 C 缺乏(HR=4.386,95％CI=2.763～6.963)和肾病综合征(HR=2.502,95％CI=1.017～6.158)对DVT复发的影响差异有统计学意义(P＜0.05).结论 性别、残余血栓、静脉曲张、蛋白C缺乏和肾病综合征是老年患者下肢DVT复发的独立危险因素.
Abstract
Objective To analyze the influencing factors of recurrence of deep vein thrombosis(DVT)in the lower extremities in elderly patients.Methods The clinical data of 500 elderly patients diagnosed with DVT of lower extremities in first visit and hospitalized in Shanxi Bethune Hospital from January 2017 to September 2018 were collected continuously.Multivariate analysis was performed using the Cox proportional hazards model to determine the influencing factors of DVT recurrence in patients.Results 85 patients had recurrence of DVT,with a recurrence rate of 17％.Cox multivariate regression analysis showed that male sex[hazard ratio(HR)=1.701,95％confidence level(CI)=1.094-2.644],residual thrombus(HR=3.033,95％CI=1.735-5.301),varicose veins(HR=1.715,95％CI=1.098-2.678),protein C deficiency(HR=4.386,95％CI=2.763-6.963)and nephrotic syndrome(HR=2.502,95％CI=1.017-6.158)had statistically significant differences in the recurrence of DVT(P＜0.05).Conclusion Gender,residual thrombus,varicose veins,protein C deficiency and nephrotic syndrome are independent risk factors for recurrence of DVT in the lower extremities in elderly patients.
