Clinical characteristics of secondary dysfunction in patients with chronic prostatitis
Objective To explore the clinical characteristics of secondary dysfunction in patients with chronic prostatitis and its influencing factors. Methods A total of 497 chronic prostatitis patients admitted to Danzhou People's Hospital from January 2019 to December 2022 were selected as study subjects. They were divided into observation group(n =202)and control group(n =295)according to whether they had secondary dysfunction. The clinical data of the two groups were compared to analyze the clinical characteristics of secondary dysfunction in patients with chronic prostatitis and the related influencing factors. Results There were no statistically significant differences in body mass index(BMI),smoking history, marital status, clinical classification, prostatic tenderness/calcification,prostatic fluid test, and sex hormone levels between the two groups(P>0.05).There were statistically significant difference in age, uncontrolled sex life, disease course, history of alcohol consumption, and pelvic floor muscle tension between the two groups(P<0.05).The scores of the national institutes of health chronic prostatitis symptom index(NIH-CPSI)and international index of erectile function-5(IIEF-5)scores in the observation group were significantly higher than those in control group, and the number of cases of anxiety and depression was significantly higher,Chinese index of premature ejaculation-5(CIPE-5)scores were significantly lower than those of the control group, and the difference was statistically significant(P<0.05).Logistic regression analysis showed that age, disease course, severity of the disease, history of alcohol consumption, white blood cell count in prostatic fluid(>10/HP)and pelvic floor muscle tension were the influencing factors of secondary dysfunction in chronic prostatitis(P<0.05),and high NHI-CPSI scores, long disease course, pelvic floor muscle tension, and anxiety were the independent risk factors of secondary dysfunction in chronic prostatitis(P<0.05).Conclusions Secondary dysfunction in patients with chronic prostatitis are mostly characterized by old age, long disease course, more serious condition, and liking to drink alcohol, and the severity of symptoms, pelvic floor muscle tension, and mental anxiety are independent risk factors for chronic prostatitis secondary dysfunction.
