目的 探讨体外受精/卵胞质内单精子注射-胚胎移植(IVF/ICSI-ET)后早期流产的相关影响因素.方法 选取2018年1月至12月中信湘雅生殖与遗传专科医院的6 107例经IVF/ICSI-ET治疗后成功妊娠患者作为研究对象,依据妊娠结局将患者分为早期流产组(n=987)和继续妊娠组(n=5 120),对比两组基线资料及临床指标,并采用二元Logistic回归分析探究早期流产的影响因素.结果 所有患者早期流产率为16.16%.单因素分析结果表明,两组年龄、窦卵泡数、移植后14d人绒毛膜促性腺激素(β-hCG)水平、基础抗米勒管激素水平、基础睾酮水平、不孕年限、助孕方式、不孕类型、有无染色体病及男方因素比较,差异具有统计学意义(P<0.05).二元Logistic回归分析结果显示,年龄、移植后14 d β-hCG水平及助孕方式是IVF/ICSI-ET患者早期流产的影响因素.结论 高龄、移植后14 dβ-hCG水平低和冻胚移植的IVF/ICSI-ET患者会增加早期流产风险.
Analysis of influencing factors of early pregnancy loss in patients undergoing in vitro fertilization-embryo transfer
Objective To investigate related influencing factors for early pregnancy loss(EPL)after in vitro fertilization/intra cytoplasmic sperm injection-embryo transfer(IVF/ICSI-ET).Methods A total of 6,107 patients with successful pregnancy after IVF/ICSI-ET treatment in Reproductive and Genetic Hospital of CITIC-Xiangya from January to December 2018 were selected as study objects.According to the pregnancy outcomes,patients were divided into the EPL group(n=987)and continued pregnancy group(n=5,120).Baseline data and clinically indicators were compared between the two groups.The influencing factors for EPL were explored by binary Logistic regression analysis.Results The EPL rate of all patients was 16.16%.Univariate analysis showed that there were statistically significant differences between the two groups in age,antral follicle counts,β-human chorionic gonadotrophin(β-hCG)level at 14-day after transfer,basal anti-Müllerian hormone(AMH)levels,basal testosterone levels,infertility years,assisted pregnancy modes,infertility types,chromosome disease and male factors(P<0.05).The results of binary Logistics regression analysis showed that age,β-hCG level at 14-day after transfer and assisted pregnancy modes were related factors to EPL in IVF/ICSI-ET patients.Conclusions IVF/ICSI-ET patients with older age,lower β-HCG at 14-day after transfer and frozen embryo transfer are at increased risk of EPL.
