Application value of 1.5T magnetic resonance imaging diffusion-weighted imaging in pathological staging of endometrial carcinoma
Objective To explore the application value of 1.5T magnetic resonance imaging(MRI)diffusion-weighted imaging(DWI)in pathological staging of endometrial carcinoma(EC).Methods A total of 82 patients with EC treated in the Wucheng District First People's Hospital from May 2019 to May 2022 were selected as study objects.According to surgical pathological staging in international federation of gynecology and obstetrics(FIGO),they were divided into stage Ⅰ group(n=39),stage Ⅱ group(n=26)and stage Ⅲgroup(n=17).All patients were examined by 1.5T MRI.Taking pathological results as the golden standard,diagnostic value of MRI-DWI for pathological staging of EC was explored.The apparent diffusion coefficient(ADC)values in patients with different staging and pathological classifications were compared.The correlation between ADC and the pathological staging was analyzed.Results There were 77 patients with consistency between MRI-DWI and pathological examination in the diagnosis of EC pathological staging with 0.903 Kappa value.Taking pathological results as the golden standard,sensitivity,specificity and accuracy of MRI-DWI in the diagnosis of EC patients at stages Ⅰ,Ⅱ and Ⅲ were(94.87%,95.56%,95.24%),(92.31%,94.64%,93.90%)and(94.12%,100.00%,98.78%),respectively.The differences in ADC values among EC patients with different staging were statistically significant(P<0.05).ADC value in patients at stage Ⅰ was(1.54±0.36)× 10-3mm2/s,which is higher than that at stages Ⅱ and Ⅲ[(1.22±0.31)× 10-3mm2/s,(0.98±0.27)× 10-3mm2/s,P<0.05)].ADC value in patients at stage Ⅱ was higher than stage Ⅲ.ADC value in patients with endometrial adenocarcinoma and non-endometrial adenocarcinoma were[(1.34±0.35)× 10-3mm2/s and(1.28±0.29)× 10-3mm2/s,respectively,with no significant difference(P>0.05).ADC value was negatively correlated with pathological staging of EC patients(P<0.05).Conclusions 1.5T MRI-DWI has high diagnostic value for pathological staging of EC.ADC value can reflect malignant degree of tumors,which can provide more accurate information for preoperative surgical examination.
