Influence of hysteroscopic endometrial polyp electrocision combined with different drug treatments on menstrual recovery status and recurrence rate in patients with endometrial polyps
Objective To explore the influence of hysteroscopic endometrial polyp electrocision combined with different drug treatments on menstrual recovery status and recurrence rate in patients with endometrial polyps.Methods A total of 102 patients with endometrial polyps in Department of Gynecology of Huainan Xinhua Medical Group Xinhua Hospital from January 2020 to October 2022 were included as study objects.All selected patients received hysteroscopic endometrial polyp electrocision,and they were divided into dydrogesterone group and levonorgestrel intrauterine system group according to the random number table method with 51 cases in each group.The menstrual recovery status,endometrial thickness,sex hormones levels and blood related biochemical indicators before and after surgery and recurrence rate and adverse reactions condition within 12 months after surgery were observed and compared in the two groups.Results The menstrual recovery time in levonorgestrel intrauterine system group was significantly shorter than that in dydrogesterone group(P<0.05).At 6 months after surgery,the number of menstrual days in levonorgestrel intrauterine system group was significantly less(P<0.05),and the menstrual cycle was significantly longer compared to dydrogesterone group(P<0.05).After surgery,the endometrial thickness in the two groups was decreased significantly(P<0.05),and the endometrial thickness were increased gradually in both groups with time(P<0.05),and the thickness in levonorgestrel intrauterine system group was significantly thinner compared with that in dydrogesterone group at each time point(P<0.05).At 6 months after surgery,the levels of luteinizing hormone(LH),estradiol(E2)and follicle stimulating hormone(FSH)were significantly decreased in the two groups(P<0.05),and the levels were significantly lower in levonorgestrel intrauterine system group than in dydrogesterone group(P<0.05).The level of C reactive protein(CRP)was decreased significantly in both groups at 6 months after surgery(P<0.05),and the levonorgestrel intrauterine system group had significantly lower CRP than dydrogesterone group(P<0.05).The levels of hemoglobin(Hb)and D-dimer were significantly increased(P<0.05),and the levels in levonorgestrel intrauterine system group were significantly higher than those in dydrogesterone group(P<0.05).The recurrence rate within 1 year was significantly lower in levonorgestrel intrauterine system group than that in dydrogesterone group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Hysteroscopic endometrial polyp electrocision combined with dydrogesterone group or levonorgestrel intrauterine system can both effectively improve the menstruation status of patients with endometrial polyps,but levonorgestrel intrauterine system has better treatment effect and lower recurrence rate.
