目的 应用广义估计方程分析含艾博韦泰方案在住院初治艾滋病患者中的疗效及影响因素.方法 回顾性收集2019年7月至2020年12月在成都市公共卫生临床医疗中心感染一科住院的使用含艾博韦泰(ABT)方案进行抗病毒治疗(ART)的120例住院初治艾滋病患者的相关资料,应用广义估计方程分析ART前后人类免疫缺陷病毒(HIV)RNA、CD4的变化及影响因素.结果 ART 2周和4周后,HIV RNA均较基线下降,CD4均较基线上升,差异均具有统计学意义(P<0.001);ART方案、合并机会性感染数及合并用药数是影响因素(P<0.05);随访检测时间节点与ART方案存在交互效应(P<0.001),ABT+整合酶方案较ABT+2核苷方案更能快速降低HIV RNA(P<0.001),但CD4的变化无显著差异(P>0.05),合并机会性感染数、合并用药数≥2种的患者ART疗效较差.结论 广义估计方程能较好地分析、探讨ART疗效及影响因素,选择优效的ART方案可提高ART疗效.
Analysis of the efficacy and multi-factor influence of albuvirtide regimens in the treatment of acquired immune deficiency syndrome patients based on the generalized estimation equations
Objective To analyze the efficacy and influencing factors of albuvirtide regimens in the treatment of hospitalized acquired immune deficiency syndrome(AIDS)patients with initial treatment based on the generalized estimation equations.Methods The relevant data of 120 hospitalized AIDS patients with initial treatment who received antiretroviral therapy(ART)with albuvirtide(ABT)regimen in infectious disease department one of Public Health Clinical Center of Chengdu from July 2019 to December 2020 were retrospectively collected.Generalized estimation equation was used to analyze the changes and influencing factors of human immunodeficiency virus(HIV)RNA,CD4 before and after ART.Results After two weeks or four weeks of ART,HIV RNA decreased significantly compared with baseline,the CD4 increased significantly compared with baseline,with statistically significan differences(P<0.001).ART regimen,co-opportunistic infection number and combined drugs number were influencing factors(P<0.05).There was an interactive effect between the follow-up detection time point and ART regimen(P<0.001).The ABT+integrase regimen reduced HIV RNA more quickly than the ABT+2 nucleoside regimen(P<0.001),but there was no significant difference in CD4 change(P>0.05).Patients with opportunistic infections or combined drugs ≥2 had poor efficacy of ART.Conclusions The generalized estimation equations can better analyze and explore the efficacy and influencing factors of ART.Choosing the optimal ART regimen can improve the efficacy of ART.
