摘要
目的 探讨特发性中枢性性早熟女童采用曲普瑞林治疗的效果和对第二性征的影响.方法 选取2021年1月至2022年1月遵化市人民医院儿科诊治的78例特发性中枢性性早熟女童作为研究对象,根据随机数字表法分为观察组和对照组,每组39例.对照组仅采用饮食调整等基础干预,观察组在对照组基础上使用曲普瑞林治疗,比较两组的治疗效果、第二性征发育指标(子宫容积、卵巢容积及乳腺大小)、生长指标[身高、体重及骨龄/实际年龄(BA/CA)]、性激素水平[卵泡刺激素(FSH)、雌二醇(E2)、黄体生成素(LH)]和药物安全性.结果 观察组治疗总有效率显著高于对照组,差异具有统计学意义(P＜0.05).治疗12个月后观察组子宫容积、卵巢容积和乳腺大小显著小于对照组,差异具有统计学意义(P＜0.05).治疗12个月后观察组体重、身高、BA/CA水平均显著高于对照组,差异具有统计学意义(P＜0.05).治疗12个月后观察组FSH、LH、E2水平显著低于对照组,差异具有统计学意义(P＜0.05).观察组仅出现2例轻微皮疹反应,药物安全性好.结论 采用曲普瑞林治疗特发性中枢性性早熟女童的临床效果好,可显著抑制性激素水平,改善第二性征发育指标和生长指标,且安全性佳.
Abstract
Objective To investigate the efficacy of treatment with triptorelin in girls with idiopathic central precocious puberty and the effects on secondary sexual characteristics.Methods A total of 78 cases of idiopathic central precocious puberty girls diagnosed and treated in the Department of Pediatric,Zunhua People's Hospital from January 2021 to January 2022 were selected as the study objects,and were divided into the observation group and the control group according to the randomized numerical table method,with 39 cases in each group.In the control group,only basic interventions such as dietary adjustments were used,and in the observation group,treatment with triptorelin was used on the basis of the control group.The treatment effects,indicators of the development of secondary sexual characteristics(uterine volume,ovarian volume and breast size),growth indicators[height,weight and bone age/actual age(BA/CA)],sex hormone levels[follicle stimulating hormone(FSH),estradiol(E2),and luteinizing hormone(LH)],and drug safety were compared between the two groups.Results The total effective rate of the treatment in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P＞0.05).Uterine volume,ovarian volume and breast size of the observation group were significantly smaller than those of the control group after 12 months of treatment,and the differences were statistically significant(P＜0.05).Weight,height and BA/CA levels of the observation group were significantly higher than those of the control group after 12 months of treatment,and the differences were statistically significant(P＜0.05).FSH,LH and E2 levels in the observation group were significantly lower than those in the control group after 12 months of treatment,and the differences were statistically significant(P＜0.05).Only 2 cases of minor rash reaction occurred in the observation group,and the drug safety is good.Conclusions The use of triptorelin in the treatment of girls with idiopathic central precocious puberty has good clinical effect.It can significantly suppress the levels of sex hormones,improve the indicators of the development of secondary sexual characteristics and growth indicators,and have a good safety profile.
