A case report of severe oligoasthenozoospermia secondary to prostatic midline cyst
Prostatic midline cyst is a clinically rare cystic lesion located in the midline region of the prostate.When the cyst compresses the ejaculatory duct,it can cause a complete or incomplete obstruction of the vas deferens,leading to abnormal semen and infertility in men.One case of severe asthenozoospermia secondary to midline prostatic cyst is reported as follows:the patient is a middle-aged male,41 years old,who sought medical attention due to"failure to conceive for more than 6 years without contraception after the partner's embryo stopped".They married in 2009,live together as husband and wife and engage in sexual activity 1-2 times per week.After the marriage,in 2009 and 2016,the spouse had 2 miscarriages after 2 months of pregnancy and underwent uterine curettage.They did not use contraception and did not conceive for more than 6 years.Routine semen testing in 2021 revealed severe oligoasthenospermia.After 6 months of intermittent medication,there was no improvement in sperm quality.A routine semen test in 2022 still showed severe oligoasthenospermia,and an ultrasound revealed a cyst in the midline of the prostate.Severe oligoasthenospermia gravis is secondary to compression of the ejaculatory duct by a midline prostatic cyst.After actively improving relevant tests to rule out surgical contraindications,ultrasound-guided puncture and drainage of prostate cysts were conducted.After surgery,oral treatment with levocarnitine was administered and semen was re-examined after 3 months.The total number,concentration and vitality of sperm improved significantly compared with before.In the clinical practice of male infertility,finding the cause is crucial.Only by identifying the cause and implementing targeted treatment can achieve better treatment outcomes.
