AMH与不同年龄多囊卵巢综合征患者性激素、糖脂代谢水平的相关性分析

目的 分析抗米勒管激素(AMH)与不同年龄多囊卵巢综合征(PCOS)患者性激素、糖脂代谢水平的相关性.方法 回顾性分析2022年1月至2023年5月在钦州市第二人民医院治疗的150例PCOS患者的临床资料,将其纳入PCOS组(n=150),根据PCOS患者年龄分为A组(年龄<25岁)、B组(25岁≤年龄<30岁)和C组(30岁≤年龄<35岁),各50例.选取同期同一医院92例体检健康女性纳入对照组.比较PCOS组与对照组、PCOS组不同年龄段分组AMH水平,比较PCOS组不同年龄分组的性激素[黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)和睾酮(T)]水平、糖脂代谢指标[空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)]及甲状腺激素[游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)和超敏促甲状腺素(TSH)]水平的差异.采用Spearman法分析AMH与PCOS组不同年龄分组的性激素、糖脂代谢水平的相关性.结果 PCOS组AMH水平高于对照组(P<0.05);随着年龄增长,FSH水平逐渐增高,AHM、LH水平逐渐降低(P<0.05);PCOS组不同年龄分组甲状腺功能比较,差异无统计学意义(P>0.05);随着年龄增长,LDL-C、HOMA-IR水平增高,HDL-C水平下降(P<0.05);AMH 与 FSH、HDL-C 及 HOMA-IR 水平呈负相关(r=-0.322、-0.490、-0.184,P<0.05),与LH、HDL-C水平呈正相关(r=0.877、0.222,P<0.05).结论 PCOS患者AMH水平高于正常人群水平,且随着年龄增长而降低,AMH与FSH、HOMA-IR、LH及HDL-C水平具有相关性.
Objective To analyze the correlation between anti-Müllerian hormone(AMH)and sex hormones,glucose-lipid metabolism in patients with polycystic ovary syndrome(PCOS)at different age groups.Methods The clinical data of 150 patients with PCOS treated in Qinzhou Second People's Hospital from January 2022 to May 2023 were retrospectively analyzed and included as PCOS group(n=150).According to PCOS patients'ages,they were divided into group A(<25 years old),group B(25 ≤ Age<30 years old)and group C(30 ≤ Age<35 years old),with 50 cases in each group.A total of 92 healthy women underwent health examination in the same hospital during the same period were enrolled as control group.AMH levels in PCOS group and control group were compared,as well as in different age groups of PCOS group.The levels of sex hormone[luteinizing hormone(LH),follicle stimulating hormone(FSH),estradiol(E2),testosterone(T)],glucose-lipid metabolism indexes[fasting plasma glucose(FPG),fasting insulin(FINS),insulin resistance index(HOMA-IR),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)]and thyroid hormones[serum free triiothyrogenine(FT3),free thyroxine(FT4),hypersensitive thyroxine(TSH)]in different age groups of PCOS group were compared.The correlation between AMH and sex hormones,glucose-lipid metabolism was analyzed by Spearman analysis.Results AMH level in PCOS group was higher than that in control group(P<0.05).With age,FSH level was increased,while AHM and LH levels were decreased(P<0.05).There was no significant difference in thyroid function among patients at different age groups of PCOS group(P>0.05).With age,LDL-C and HOMA-IR levels were increased,while HDL-C level was decreased(P<0.05).AMH level was negatively correlated with FSH,HDL-C and HOMA-IR levels(r=-0.322,-0.490,-0.184;P<0.05),while positively correlated with LH and HDL-C levels(r=0.877,0.222;P<0.05).Conclusions Compared with normal people,AMH level is higher in PCOS patients,and it decreases with age.AMH level is correlated with FSH,HOMA-IR,LH and HDL-C levels.
