Analysis of the effectiveness of low-intensity extracorporeal shock wave therapy for chronic prostatitis/chronic pelvic pain syndrome and its early predictive factors
Objective To explore the clinical efficacy of low-intensity extracorporeal shock wave therapy(Li-ESWT)in patients with chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS).Methods From June 2022 to January 2023,200 CP/CPPS patients treated with Li-ESWT in the Second Affiliated Hospital of Xinjiang Medical University were selected as the research subjects.The therapeutic effect was evaluated using the national institutes of health chronic prostatitis symptom index(NIH-CPSI).Expressed prostate secretion(EPS)was collected at different time points to detect the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and interleukin-1 β(IL-1β)in EPS.Logistic regression analysis was used to analyze the factors independently associated with the therapeutic effect of Li-ESWT.The nomogram model was established using R software,and the receiver operating characteristic(ROC)curve was used to evaluate the internal efficacy of the model in early prediction of the therapeutic effect of Li-ESWT.Results After 4 cycles of treatment,the total score of NIH-CPSI,pain discomfort score,urinary symptom score,quality of life score,and the levels of IL-6,TNF-α,and IL-1β in EPS were all lower than those after 2 cycles of treatment,with statistically significant differences(P<0.05).The Logistic regression analysis showed that the pre-treatment NIH-CPSI score and age were independent protective factors for poor efficacy of Li-ESWT(P<0.05).The history of alcohol consumption,long-term urinary retention habits,and sexual life more than twice a week were independent risk factors for poor efficacy of Li-ESWT(P<0.05).The ROC curve results of the nomogram model showed that the area under the curve(AUC)for early prediction of poor efficacy of Li-ESWT was 0.918,with sensitivity of 82.76%and specificity of 85.21%.Conclusions Li-ESWT treatment for CP/CPPS patients can effectively reduce the NIH-CPSI score,alleviate prostatitis inflammation,and improve clinical symptoms.The predictive model constructed based on the related factors has high clinical value in early assessment of the therapeutic effect of Li-ESWT in CP/CPPS.
