目的 探讨精浆中微小RNA-188-3p(miR-188-3p)、微小RNA-4485-3p(miR-4485-3p)水平对非梗阻性无精子症(NOA)患者的临床诊断价值。方法 选取2021年11月至2023年11月秦皇岛市第一医院诊治的83例NOA患者作为NOA组,91例弱精子症患者作为弱精子症组,87例同期各项指标正常的志愿者作为正常组。对三组的基线资料及精浆中miR-188-3p、miR4485-3p水平进行比较,采用Logistic回归分析NOA发生的影响因素,采用受试者工作特征(ROC)曲线分析精浆中miR-188-3p、miR-4485-3p水平对NOA的诊断价值。结果 NOA组泌尿生殖系统感染史患者占比高于弱精子症组和正常组(P<0。05);NOA组精浆中miR-188-3p、miR-4485-3p水平低于弱精子症组和正常组(P<0。05);精浆中 miR-188-3p、miR-4485-3p 是 NOA 发生的保护因素(P<0。05);精浆中 miR-188-3p、miR-4485-3p 联合诊断 NOA 的效能高于各项指标单独诊断(Z联合-miR-188-3p=4。682,P<0。001;Z联合-miR-4485-3p=2。743,P=0。006)。结论 NOA患者精浆中miR-188-3p、miR-4485-3p水平显著下降,且精浆中miR-188-3p联合miR-4485-3p诊断NOA的效能高于各指标单独诊断。
Clinical diagnostic value of microRNA-188-3p and microRNA-4485-3p levels in seminal plasma for non-obstructive azoospermia patients
Objective To explore the clinical diagnostic value of microRNA-188-3p(miR-188-3p)and microRNA-4485-3p(miR-4485-3p)levels in seminal plasma for patients with non-obstructive azoospermia(NOA).Methods A total of 83 NOA patients admitted to the Qinhuangdao First Hospital from November 2021 to November 2023 were selected as NOA group,91 patients with asthenospermia were selected as asthenospermia group,and 87 volunteers with normal indexes in the same period were selected as normal group.The baseline data and the levels of miR-188-3p and miR-4485-3p in seminal plasma were compared among three groups,Logistic regression was applied to analyze the influencing factors of NOA occurrence,receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of miR-188-3p and miR-4485-3p levels in seminal plasma for NOA.Results The proportion of patients with urinary and reproductive system infections history in the NOA group was higher than that in the asthenospermia group and normal group(P<0.05);the levels of miR-188-3p and miR-4485-3p in the seminal plasma of the NOA group were lower than those of the asthenospermia group and the normal group(P<0.05);miR-188-3p and miR-4485-3p in seminal plasma were protective factors for the occurrence of NOA(P<0.05).The combined diagnostic efficacy of miR-188-3p and miR-4485-3p in seminal plasma for NOA was higher than that of each indicator alone(Zcombination-miR-188-3p=4.682,P<0.001;Zcombination-miR-4485-3p=2.743,P=0.006).Conclusions The levels of miR-188-3p and miR-4485-3p in the seminal plasma of NOA patients are significantly decreased,and the diagnostic efficacy of miR-188-3p combined with miR-4485-3p in the seminal plasma for NOA was higher than that of each indicator alone.
