短链脂肪酸与糖尿病认知功能障碍

随着糖尿病发病率的逐年上升,糖尿病及其并发症受到越来越多的重视.糖尿病认知功能障碍(diabetic cognitive impairment,DCI)作为糖尿病的神经系统并发症,近年来发病率呈显著上升趋势,其相关机制有待阐明.短链脂肪酸(short-chain fatty acids,SCFAs)是肠道菌群发酵膳食纤维的代谢产物,在"微生物-肠-脑轴"中起重要作用,近年来与糖尿病及大脑功能之间的联系逐渐成为研究热点.探索SCFAs与DCI之间的关系尤为重要.本文综述了 SCFAs在糖尿病、认知功能障碍及DCI人群中的变化特征,并梳理了 SCFAs在DCI中的作用机制,即糖脂代谢、病理性蛋白聚集、炎性反应及线粒体自噬.未来的研究有必要针对糖尿病和认知功能障碍共同的病理生理学机制开展相应的机制研究,以探索不同种类、不同剂量的SCFAs在DCI中的效果及潜在机制,从而为防治"共病模式"背景下疾病的发生与发展提供一定的理论依据.
Short-chain fatty acids and diabetic cognitive impairment
Diabetes mellitus and its consequences have gained increased attention with the incidence rate rising.Diabetic cognitive impairment(DCI),a neurological consequence of diabetes mellitus,has shown a significant increase in recent years,and its related mechanisms need to be elucidated.Short-chain fatty acids(SCFAs),which are metabolites of dietary fiber fermented by gut microbiota and play a significant role in the"microbial-gut-brain axis",have become a popular research topic in recent years for their link to dia-betes mellitus and brain function.Therefore,it is important to explore the relationship between SCFAs and DCI.This review summarized the changing characteristics of SCFAs in diabetes mellitus,cognitive impair-ment and DCI populations,as well as the mechanisms of SCFAs in DCI,which include glucose-lipid metabo-lism,pathological protein aggregation,inflammation,and mitochondrial autophagy.Future studies should ad-dress the common pathophysiological mechanisms of diabetes mellitus and cognitive impairment,in order to explore the effects of different types and doses of SCFAs in DCI and the underlying mechanisms,so as to pro-vide a theoretical basis for the prevention and treatment of the disease in the context of the"co-morbidity model".
