摘要

目的 分析血清中基质金属蛋白酶-9(MMP-9)与大动脉粥样硬化型脑卒中(LAA)患者颈动脉斑块新生血管的相关性.方法 选取120 例经常规超声检查发现颈动脉粥样硬化斑块形成的患者作为研究对象,并根据患者在72 h内是否发生急性缺血性脑卒中分为实验组和对照组,两组各60 例.比较两组患者不同颈动脉斑块狭窄程度、斑块回声、新生血管情况,并通过SMI检测颈部斑块新生血管情况,并分析血清中MMP-9水平与大动脉粥样硬化型脑卒中患者颈动脉斑块新生血管的关系,分析颈动脉斑块特征及相关危险因素与大动脉粥样硬化型脑梗死之间的关系.结果 实验组斑块内新生血管高于对照组(P<0.05);有新生血管组UA、Lp(a)、MMP-9水平升高及糖尿病患者人数均高于无新生血管组(P<0.05)且MMP-9为主要因素.结论 血清中MMP-9水平与斑块内新生血管形成密切相关,且斑块内新生血管与LAA发病率相关.

Abstract

Objective To analyze the correlation between matrix metalloproteinase-9(MMP-9)in serum and carotid plaque neovascularization in patients with large artery atherosclerotic stroke(LAA).Methods A total of 120 patients with carotid atherosclerotic plaque formation found by routine ultrasound examination were selected as the study subjects,and according to whether the patients had acute ischemic stroke within 72 hours,they were divided into experimental group and control group,with 60 cases in each group.The degree of carotid plaque stenosis,plaque echo and neovascularization were compared between the two groups,and the neovascularization of cervical plaque was detected by SMI,and the relationship between serum MMP-9 level and carotid plaque neovascularization in patients with aortic atherosclerotic stroke was analyzed,and the relationship between carotid plaque characteristics and related risk factors and aortic atherosclerotic cerebral infarction was analyzed.Results The neovascularization in the experimental group was higher than that in the control group(P<0.05).The levels of UA,Lp(a)and MMP-9 in the neovascular group were higher than those in the non-neovascular group(P<0.05),and MMP-9 was the main factor.Conclusion Serum level of MMP-9 is closely related to intraplaque neovascularization,and intraplaque neovascularization is associated with the incidence of LAA.
