摘要
目的 观察外周血T淋巴细胞亚群、凝血指标与急性白血病患者病情的相关性.方法 选取2019年3月—2022年3月在商丘市第一人民医院收治的73 例急性白血病患者作为观察组,并按1:1配比原则选择同期73 名健康体检者作为对照组.比较2组T淋巴细胞亚群、凝血指标[纤维蛋白原(FIB)、D-二聚体、凝血酶原时间(PT)],并分析不同危险度分层及不同预后T淋巴细胞亚群、凝血指标,分析T淋巴细胞亚群与凝血指标相关性.结果 观察组CD3+、CD4+、FIB均低于对照组,CD8+、D-二聚体、TT均高于对照组(P＜0.05);高危CD3+、CD4+、FIB＜中危＜低危,高危CD8+、D-二聚体、TT＞中危＞低危(P＜0.05);CD3+、CD4+与D-二聚体、TT呈负相关,与FIB呈正相关(P＜0.05);CD8+与D-二聚体、TT呈正相关,与FIB呈负相关(P＜0.05);预后不良治疗前后CD3+、CD4+、FIB均低于预后良好,CD8+、D-二聚体、TT均高于预后良好(P＜0.05).结论 急性白血病外周血T淋巴细胞亚群、凝血指标与患者病情密切相关,可有助于临床病情监测和预后评估.
Abstract
Objective To observe the correlation between peripheral blood T lymphocyte subsets and coagulation index and the condition of patients with acute leukemia.Methods From March 2019 to March 2022,73 patients with acute leukemia admitted to Shangqiu First People's Hospital were selected as the observation group,and 73 healthy physical examiners in the same period were selected as the control group according to the principle of 1:1 ratio.The two groups of T lymphocyte subsets and coagulation indexes[fibrinogen(FIB),D-dimer,prothrombin time(PT)]were compared,and different risk stratification and different prognostic T lymphocyte subsets and coagulation indexes were analyzed,and the correlation between T lymphocyte subsets and coagulation indexes was analyzed.Results CD3+,CD4+,and FIB in the observation group were lower than those in the control group,CD8+,D-dimer and TT were higher than those in the control group(P＜0.05).High-risk CD3+,CD4+,FIB＜intermediate-risk＜low-risk,high-risk CD8+,D-dimer,TT＞intermediate-risk＞low-risk(P＜0.05),CD3+and CD4+were negatively correlated with D-dimer and TT,and positively correlated with FIB(P＜0.05).CD8+were positively correlated with D-dimer and TT.It was negatively correlated with FIB(P＜0.05).CD3+,CD4+,and FIB were lower than those with good prognosis before and after treatment,and CD8+,D-dimer and TT were higher than those with good prognosis(P＜0.05).Conclusion Peripheral blood T lymphocyte subsets and coagulation indexes in acute leukemia are closely related to patients'disease conditions,which can be helpful for clinical disease monitoring and prognosis evaluation.
