摘要

目的 探究盐酸氟桂利嗪治疗偏头痛性眩晕对患者的临床疗效.方法 选取2021年3月—2023年3月永州市第三人民医院神经内科收治的偏头痛性眩晕患者104 例并应用纸牌分组法随机分为观察组及对照组各52 例,比较两组患者不良反应,于治疗前及治疗30 d后比较两组患者临床症状改善情况(头痛发作次数、头痛持续时间、每月无头痛天数)、血清活性肽[血清降钙素基因相关酶(CGRP)、垂体腺苷酸环化酶激活肽(PACAP)、中枢神经特异蛋白(S100B)]、血管内皮活性物质[内皮素-1(ET-1)、一氧化氮(NO)、5-羟色胺(5-HT)]、脑血流动力学[大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)]水平变化.结果 治疗30 d后,观察组患者每月无头痛天数显著高于同期对照组(P<0.05);临床症状缓解时间(头痛发作次数、头痛持续时间)、血清活性肽、血管内皮活性物质、脑血流动力学指标均显著低于同期对照组(P<0.05).对照组患者出现1 例恶心,观察组患者出现1 例嗜睡,其余患者均无明显不良反应.结论 盐酸氟桂利嗪能够调节偏头痛性眩晕患者脑部血流动力学,改善患者血管内皮功能,减轻患者临床症状,且不良反应较少,临床疗效良好.

Abstract

Objective To explore the clinical efficacy of Flunarizine Hydrochloride in the treatment of migraine vertigo.Methods 104 patients with migraine vertigo admitted to Neurology Department of Third People's Hospital of Yongzhou City from March 2021 to March 2023 were randomly divided into observation group and control group with 52 patients in each group by card grouping method.Adverse reactions were compared between the two groups.The improvement of clinical symptoms(number of headache attacks,duration of headache,number of headache free days per month),changes of serum active peptide[serum calcitonin gene-related enzyme(CGRP),pituitary adenylate cyclase activating peptide(PACAP),central nervous specific protein(S100B)],vascular endothelial active substances[endothelin-1(ET-1),nitric oxide(NO),5-hydroxytryptamine(5-HT)],and cerebral hemodynamics[anterior cerebral artery(ACA),middle cerebral artery(MCA),posterior cerebral artery(PCA)]were compared between the two groups before treatment and 30 days after treatment.Results After 30 days of treatment,the number of headache free days per month in observation group was significantly higher than that in control group(P<0.05).The remission time of clinical symptoms(the number of headache attacks and the duration of headache),serum active peptide,vascular endothelial active substances and cerebral hemodynamics indexes were significantly lower than those in the control group(P<0.05).There was 1 case of nausea in the control group,1 case of drowsiness in the observation group,and no significant adverse reactions in other patients.Conclusion Flunarizine Hydrochloride can regulate cerebral hemodynamics,improve vascular endothelial function and alleviate clinical symptoms in patients with migraine vertigo,with fewer adverse reactions and good clinical efficacy.
