综合三级医院医务人员健康体检血脂水平调研分析

摘要
目的 调研医务人员血脂分布水平,分析伴随其他生化指标异常的情况,为医务人员系统性的血脂健康管理提供依据.方法 回顾性统计分析4 782 名在职医务人员年度健康体检的生化资料,通过男女分类与年龄分层进行多维度分析血脂分布水平,及血脂水平异常合并其他生化指标的异常情况.结果(1)总体血脂异常检出率43.0%(2 057/4 782),其中男性异常率65.2%(832/1 276)明显高于女性异常率34.9%(1 225/3 506),血脂异常率整体有伴随年龄增加而升高的趋势.(2)男性组血脂异常率升高年龄段早于女性,男性在31-40 岁开始明显升高,到41-50 岁达到高峰且血脂异常率一直维持在较高水平;而女性于41-50 岁才开始明显升高,在51-60 岁升高幅度最大.各年龄组比较,除≥61 岁年龄组外,其他年龄段男性血脂异常率均高于女性;但>50 岁之后,男女血脂异常率差异逐渐缩小.(3)血脂四项TC、TG、LDL-C、HDL-C的异常水平在不同年龄段人群分布存在差异,在50 岁前,TC、LDL-C的异常频率男性均高于女性;50 岁后,女性TC、LDL-C的异常频率超过男性,而TG、HDL-C项目在各年龄段男性的异常频率均明显高于女性.随着年龄段增长,TC、TG、LDL-C异常分布水平呈现先升高再降低的变化趋势,而HDL-C是降低后持续低水平状态;TG与HDL-C在年龄段组内差异有统计学意义(P<0.05).(4)血脂异常组伴随ALT、AST、GGT、UREA、Cr-S、UA、GLU异常发生率均高于血脂正常组,且在GGT、Cr-S、UA、GLU项目检测水平中差异有统计学意义(P<0.05).结论 医务人员存在较严重的血脂异常现象,且在不同年龄段男女人群分布差异较大,应采取针对性的健康管理措施,以提高医疗行业人员的整体健康水平.
Abstract
Objective To investigate the distribution level of blood lipids in medical staffand analyze the abnormal situation accompanied by other biochemical indicators,so as to provide evidence for the systematic management of blood lipids in medical staff.Methods The biochemical data of 4 782 active medical personnel in annual physical examination were retrospectively analyzed.The distribution of multidimensional lipid levels and abnormal lipid levels combined with other biochemical indicators were analyzed by gender classification and age stratification.Results(1)The overall detection rate of dyslipidemia was 43.0%(2 057/4 782),of which 65.2%(832/1 276)in males was significantly higher than 34.9%(1 225/3 506)in females.The overall rate of dyslipidemia increased with the increase of age.(2)The rate of dyslipidemia in male group increased earlier than that in female group.The rate of dyslipidemia in male group increased significantly from 31-40 years old to 41-50 years old,and the rate of dyslipidemia remained at a high level.In women,the increase began to be obvious at the age of 41-50,and the increase was the largest during the age of 51-60.The rate of dyslipidemia in males was higher than that in females except≥61 years old.However,after the age of>50 years,the difference in the rate of dyslipidemia between men and women gradually narrowed.(3)Abnormal levels of TC,TG,LDL-C and HDL-C in blood lipids were different in different age groups.Abnormal frequencies of TC and LDL-C were higher in males than in females before the age of 50.After the age of 50,the abnormal frequency of TC and LDL-C in women was higher than that in men,while the abnormal frequency of TG and HDL-C items in men at all ages was significantly higher than that in women.With the increase of age,the abnormal distribution levels of TC,TG and LDL-C showed a trend of first increasing and then decreasing,while HDL-C remained at a low level after decreasing.There was statistical difference between TG and HDL-C in age group(P<0.05).(4)The incidence of abnormal ALT,AST,GGT,UREA,Cr-S,UA and GLU in dyslipidemia group was higher than that in normal lipid group,and there were statistical differences in the detection levels of GGT,Cr-S,UA and GLU(P<0.05).Conclusion There are serious dyslipidemia in medical personnel,and the distribution of male and female groups in different age groups is quite different.Targeted health management measures should be taken to improve the overall health level of medical personnel.
