Objective To investigate and analyze the distribution characteristics of serum total bile acid(TBA)metabolism in pregnant women in order to provide a strong basis for the effective prevention of intrahepatic cholestasis of pregnancy(ICP).Methods A cross-sectional survey was conducted among pregnant women admitted to the Department of Obstetrics and Gynecology of First Affiliated Hospital of Soochow University in 2023,and clinical case data such as TBA,liver function and age were collected.SPSS Statistics 26.0 statistical software was used to analyze the distribution characteristics and related influencing factors of TBA level in pregnant women.Results A total of 7 546 pregnant women aged(30.46±4.05)years were included in this study,and 175(2.32%)pregnant women with high serum TBA were detected,with statistically significant differences among pregnant women of different ages(P<0.001),with the highest detection rate in the 31-34 age group(3.59%).The average abnormal level of TBA was(23.36±16.17)μmol/L,and pregnant women with relatively higher serum TBA were mainly distributed in the age group of 26-34 years,among which the level of TBA was the highest in the age group of 26-30 years,but the difference between different ages was not statistically significant(P>0.05).ICP was detected differently in different age groups,and the distribution difference was statistically significant(P<0.05),including 153 cases(87.43%)of mild type,21 cases(12.00%)of severe type and 1 case(0.57%)of extremely severe type,mainly concentrated in pregnant women aged 31-34 years.In various types,the proportion was 50.98%(78/153)of mild type,47.62%(10/21)of severe type and 100%(1/1)of extremely severe type.The TBA level of ICP in other BMI groups was higher than that in normal BMI group,mainly distributed in underweight and obese people,among whom obese people were the most significant,but the difference in TBA level among different BMI groups was not statistically significant(P>0.05).Conclusion Obstetricians need to expand the screening range of pregnant women with high serum TBA,strengthen TBA monitoring in high-risk age groups,alert to the occurrence and progression of ICP,and reduce the adverse pregnancy outcome of ICP.
