摘要

目的 探讨血清PCT、IL-1β、IL-8在过敏性鼻炎(AR)患儿中的表达及与鼻通气功能的相关性.方法 选取2022年3月—2023年3月商丘市中医院收治的106 例AR患儿,根据病情程度分为轻度组(n=41)和中重度组(n=65),同期选取健康体检且确认非过敏性鼻炎儿童53 名为对照组.比较3组血清PCT、IL-1β、IL-8,比较2组鼻通气功能[鼻呼吸量(NS)、鼻气道阻力(NAR)、鼻腔最小横截面积(NMCA)、0-5 cm鼻腔容积(NCV0-5)]、分析血清PCT、IL-1β、IL-8与病情程度、鼻通气功能的相关性及对AR诊断价值.结果 PCT、IL-1β、IL-8两两比较:中重度组>轻度组>对照组(P<0.05).中重度组SFAR、VAS评分高于轻度组(P<0.05).中重度组NS低于轻度组,NAR高于轻度组,NCV0-5、NMCA小于轻度组,差异有统计学意义(P<0.05).IL-1β、IL-8单独诊断AR的AUC分别为0.718(95%CI 0.676~0.762)、0.732(95%CI 0.680~0.789),最佳截断值分别为42.10 ng/L、10.79 ng/L,联合诊断AR的AUC为0.820,特异度为86.79%,敏感度为84.91%,明显高于各血清指标单独诊断(P<0.05).IL-1β、IL-8与NS、NMCA、NCV0-5呈负相关,与NAR及SFAR、VAS评分呈正相关(P<0.05).结论 AR患儿血清IL-1β、IL-6表达大幅上调,二者与病情严重程度及鼻通气功能均有一定相关性,联合检测可作为临床早期诊断的重要辅助途径.

Abstract

Objective To investigate the expression of serum PCT,IL-1β and IL-8 in children with allergic rhinitis(AR)and their correlation with nasal ventilation function.Methods 106 children with AR admitted to Shangqiu Hospital of Traditional Chinese Medicine from March 2022 to March 2023 were divided into mild group(n=41)and moderate to severe group(n=65)according to the severity of the disease.During the same period,53 children with non-allergic rhinitis confirmed by physical examination were selected as the control group.Serum PCT,IL-1β,IL-8 in 3 groups were compared.The nasal ventilation function[nasal breathing volume(NS),nasal airway resistance(NAR),nasal minimum cross-sectional area(NMCA),0-5 cm nasal volume(NCV0-5)]of the two groups were compared,and the correlation between serum PCT,IL-1β,IL-8 and the degree of disease and nasal ventilation function was analyzed,and the diagnostic value of AR was also analyzed.Results PCT,IL-1β,IL-8 pairwise comparison:moderate to severe group>mild group>control group(P<0.05).SFAR and VAS scores in moderate to severe group were higher than those in mild group(P<0.05).NS in moderate to severe group was lower than that in mild group,NAR was higher than that in mild group,NCV0-5 and NMCA were lower than that in mild group,and the differences were statistically significant(P<0.05).The AUC of IL-1β and IL-8 in the diagnosis of AR were 0.718(95%CI 0.676-0.762)and 0.732(95%CI 0.680-0.789),respectively,and the optimal cut-offvalues were 42.10 ng/L and 10.79 ng/L,respectively.The AUC of combined diagnosis of AR was 0.820,the specificity was 86.79%,and the sensitivity was 84.91%,which was significantly higher than that of single diagnosis of each serum index(P<0.05).IL-1β and IL-8 were negatively correlated with NS,NMCA and NCV0-5,but positively correlated with NAR,SFAR and VAS scores(P<0.05).Conclusion The expression of serum IL-1β and IL-6 in children with AR is significantly up-regulated,and both are correlated with the severity of the disease and nasal ventilation function.Combined detection can be an important auxiliary way for early clinical diagnosis.
