摘要
目的 分析孕晚期血清胎盘蛋白-13(PP-13)、富亮氨酸α-2糖蛋白-1(LRG1)、摄食抑制因子-1(NSF-1)水平对妊娠期糖尿病(GDM)患者妊娠结局的预测价值.方法 选取2021年7月—2023年6月开封市中心医院GDM患者为研究组(n=60)及同期健康孕检者为对照组(n=60);根据患者妊娠结局分为良好妊娠、不良妊娠患者.对比2组孕晚期血清PP-13、LRG1、NSF-1水平;单因素分析GDM患者妊娠不良结局;多因素分析GDM患者妊娠不良风险,并分析血清各指标水平对患者妊娠结局的预测价值.结果 研究组血清PP-13水平低于对照组,血清NSF-1、LRG1水平均高于对照组(P<0.05);与妊娠良好患者相比,妊娠不良患者血清HbA1c、NSF-1、LRG1水平显著升高,血清PP-13水平显著降低(P<0.05);孕晚期时血清HbA1c、NSF-1、LRG1为患者发生妊娠不良的危险因素、血清PP-13为患者发生妊娠不良的保护因素(P<0.05);孕晚期时血清PP-13、LRG1、NSF-1水平联合预测患者发生妊娠不良的曲线下面积(AUC)为0.831.结论 GDM患者中血清PP-13、LRG1、NSF-1水平发生较大变化,其联合检测对患者妊娠结局具有较高的预测价值,在临床上可加强相关监测,制定合理的治疗措施干预妊娠结局.
Abstract
Objective To analyze the predictive value of serum placental protein-13(PP-13),leucine-rich α-2 glycoprotein-1(LRG1),and food inhibitor-1(NSF-1)levels in the third trimester of pregnancy in patients with gestational diabetes mellitus(GDM).Methods GDM patients in Kaifeng Central Hospital were selected as the study group(n=60)and healthy pregnant women in the same period from July 2021 to June 2023 were selected as the control group(n=60).According to the pregnancy outcome,the patients were divided into good pregnancy and bad pregnancy.The levels of serum PP-13,LRG1 and NSF-1 in late pregnancy were compared between the two groups.Univariate analysis was used to analyze the adverse outcome of pregnancy in GDM patients.The risk of adverse pregnancy in GDM patients was analyzed by multivariate analysis,and the predictive value of serum indexes on pregnancy outcome was analyzed.Results The levels of serum PP-13 in the study group were lower than those in the control group,and the levels of serum NSF-1 and LRG1 were higher than those in the control group(P<0.05).Compared with patients with good pregnancy,the serum levels of HbA1c,NSF-1 and LRG1 in patients with poor pregnancy were significantly increased,and the serum levels of PP-13 were significantly decreased(P<0.05).In the third trimester,serum HbA1c,NSF-1 and LRG1 were the risk factors for adverse pregnancy,and serum PP-13 was the protective factor for adverse pregnancy(P<0.05).The area under the curve(AUC)of the combined serum PP-13,LRG1,and NSF-1 levels in the third trimester of pregnancy was 0.831.Conclusion The serum levels of PP-13,LRG1 and NSF-1 in patients with GDM are significantly changed,and the combined detection of PP-13,LRG1 and NSF-1 has a high predictive value for the pregnancy outcome of patients,which can be strengthened in clinical monitoring and reasonable treatment measures to intervene in pregnancy outcome.
