摘要

肾脏是高尿酸血症的主要累及器官之一,高尿酸血症性肾病是高尿酸血症临床常见的并发症之一.由于血尿酸水平随肾小球滤过率的下降而升高,既往高尿酸血症被认为是肾脏损伤的标志物.最近研究表明高尿酸血症是肾脏疾病发生与进展的独立危险因素,高尿酸血症与肾脏损伤的因果关系一直存在争议.近年来,越来越多的研究证实高尿酸血症可通过诱发氧化应激、炎症反应、内皮功能障碍、肾纤维化等机制导致肾脏损伤.该文以尿酸的代谢、高尿酸血症的病因为出发点,对高尿酸血症性肾病发病机制的研究进展作一综述.

Abstract

Kidney is one of the main organs involved in hyperuricemia,and hyperuricemic nephropathy is one of the common clinical complications of hyperuricemia.Since serum uric acid levels increase with the decrease of glomerular filtration rate,hyperuricemia was previously considered a marker of renal injury.Recent studies have shown that hyperuricemia is an independent risk factor for the development and progression of renal disease,and the causal relationship between hyperuricemia and renal injury has been controversial.In recent years,more and more studies have shown that hyperuricemia can lead to renal injury by inducing oxidative stress,inflammatory response,endothelial dysfunction,renal fibrosis and so on.Based on the metabolism of uric acid and the etiology of hyperuricemia,this article reviews the research progress on the pathogenesis of hyperuricemic nephropathy.
