Epidemiological investigation of 621 inpatients with electrical burn
Objective To investigate and analyze the epidemiological characteristics of inpatients with electrical burn in Beijing Jishuitan Hospital(authors'unit),and further to provide reference guide for the prevention and treatment of electrical burn.Method The medical records of inpatients suffered from burns and electrical burns in authors'unit from January 2010 to December 2022 were collected.The patients'gender,age,population category,injury context,injury season,total body surface area(TBSA),depths of wounds,electrical voltages of injury,sites of wounds,treatment method,amputation rates,hospital length of stay,hospitalization expenses and treatment outcome were collected and statistical analyses were performed.Result During the 13 years,3089 burn patients were admitted.Among them,621 patients(20.1%)with electrical burn.There were 432 patients(69.6%)with electrical contact burns and 189 patients(30.4%)with electrical arc burns.Among them,592 were male and 29 were female,the mean age was(35±13)years old,and 77.62%(482/621)of patients were 21-50 years old.The top three groups in population category were workers,peasants and students,and the proportion was 49.11%(305/621),33.33%(207/621)and 5.64%(35/621),respectively.The non-electric professional jobs ranked the first in the proportion of injury context,which composed 46.70%(290/621)of all victims.The injury occurs mostly in summer 42.67%(265/621)and autumn 23.99%(149/621).There were 55.23%(343/621)of the patients with burn area less than 10%TBSA.The percentage of high-voltage electrical contact burn patients was 69.54%,which was obviously higher than 15.25%of the electrical arc burn patients(χ2=62.893,P<0.001).The most common wounds site of electrical burn patients was upper limbs(60.49%),followed by lower limbs(16.24%),head and neck(11.76%),trunk(9.72%),hip and perineum(1.79%).The operation rate of electrical burn patients was 89.86%,obviously higher than 71.31%of the nonelectrical burn patients(χ2=91.078,P<0.001).Wounds of 176 electrical contact burn patients were repaired with free flap by vascular anastomosis,which was 31.54%in the operation rate of electrical burn patients.The total amputation rate was 16.91%,the amputation rate of electrical burn patients was 23.15%,obviously higher than 2.65%of the electrical arc burn patients(χ2=39.339,P<0.001).The number of operations,hospital length of stay,hospitalization expenses of electrical burn patients were 2(1,4),27(16,45)d,75445.00(32213.50,132249.50)Yuan.Compared with those of electrical arc burn patients,the number of operations,hospital length of stay,hospitalization expenses were obviously higher than those of electrical contact burn patients(P<0.001).Conclusion Men are more likely than women to suffer from electric burns in authors'unit.Young workers and peasants may be the important people to prevent electric burns.It is very important to strengthen the management of electric safety education for people and to warn and protect the surrounding area of electric environment,especially in summer and autumn.The patients with electric burn may benefit from early and active debridement,and using tissue flaps including free flap to repair of wounds.
