Analysis of the mediating effect of subjective physical fatigue between muscle strength and fall risk in elderly patients
Objective To explore the mediating effect of subjective physical fatigue between muscle strength and fall risk in elderly patients.Method The elderly patients hospitalized in the Department of Geriatrics of Beijing Friendship Hospital,Capital Medical University from January 2019 to December 2022 were selected as the study objects.167 patients with a history of fall one year before admission were selected as the case group,and 167 patients with no history of fall one year before admission were selected as the control group.The clinical features in the two groups were compared and analyzed.To analyze the correlation between subjective physical fatigue,muscle strength and fall risk in elderly patients.The independent influencing factors of fall risk in elderly patients were analyzed.To analyze the mediating effect of subjective physical fatigue between muscle strength and fall risk in elderly patients.Result The body mass index,muscle strength in the case group were lower than those in control group,and the age,the proportion of subjective physical fatigue were higher than those in control group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that subjective physical fatigue was significantly negatively correlated with muscle strength(r=-0.243,P<0.05),and significantly positively correlated with fall risk(r=0.160,P<0.05).There was significant negative correlation between muscle strength and fall risk in elderly patients(r=-0.240,P<0.05).Multivariate logistic regression analysis showed that body mass index and muscle strength were independent protective factors for fall risk in elderly patients(P<0.05),and subjective physical fatigue was independent risk factor for fall risk in elderly patients(P<0.05).The results of Model 4 intermediary model analysis showed that muscle strength had a significant predictive effect on fall risk(β=0.132,t=4.311,P=0.000),and when the mediating variable was added,the direct predictive effect of muscle strength on fall risk was still significant(β=0.121,t=2.103,P=0.000).Muscle strength was significant in predicting subjective physical fatigue(β=0.219,t=3.151,P=0.000),and subjective physical fatigue was significant in predicting fall risk(β=0.052,t=3.912,P=0.000).The results of Bootstrap test model analysis showed that the direct effect value of muscle strength on fall risk was 0.641(95%CI 0.615-0.853),and the mediating effect value of subjective physical fatigue was 0.326(95%CI 0.032-0.221),and 95%CI did not contain 0,indicating that the mediating effect was established.The intermediate effect accounted for 33.72%of the total effect.Conclusion Subjective physical fatigue is the mediating variable between muscle strength and fall risk in elderly patients.Clinical evaluation and intervention of subjective physical fatigue in elderly patients should be paid more attention,so as to help reduce the fall risk of elderly patients.
