Prognostic value of patients with gastric cancer with peritoneal metastasis based on double contrast-enhanced ultrasonography parameters and prognostic nutrition index
Objective To investigate the prognostic value of double contrast-enhanced ultrasonography(DCEUS)parameters and prognostic nutrition index(PNI)in predicting the prognosis of patients with gastric cancer with peritoneal metastasis.Method 90 cases of gastric cancer with peritoneal metastasis in the People's Hospital of Kizilsu Kirgiz Autonomous Prefecture from January 2021 to January 2022 were selected as the observation group,and 90 cases of gastric cancer patients without peritoneal metastasis in the People's Hospital of Kizilsu Kirgiz Autonomous Prefecture during the same period were selected as the control group.DCEUS parameters[baseline intensity(BI),peak intensity(PI),rise time(RT),time to peak(TTP),enhanced intensity(EI)]and PNI of the two groups were measured and compared,and the relationship between the above parameters,PNI and clinicopathological factors and their prognostic value were analyzed.Result BI,PI and EI in observation group were higher than those in control group,while RT,TTP and PNI in observation group were lower than those in control group(P<0.05).BI,PI,EI,RT and TTP were correlated with tumor T stage,peritoneal metastasis grade,ascites or not,serum CEA and CA19-9 levels,and palliative gastrectomy combined with postoperative chemotherapy score(P<0.05).PNI was correlated with peritoneal metastasis grade,Hb,PLR,NLR,ALB and palliative gastrectomy combined with postoperative chemotherapy score of patients with gastric cancer and peritoneal metastasis(P<0.05).The median survival time of 90 patients with gastric cancer with peritoneal metastasis was 11.7 months,and the 1-year survival rate was only 47.78%.Kaplan-Meier curve analysis showed that the 1-year survival rate of patients with high level of BI,PI and EI was lower than that of patients with low level,and the 1-year survival rate of patients with high level of RT,TTP and PNI was higher than that of patients with low level(P<0.05).Multivariate Cox regression analysis showed that tumor T stage,peritoneal metastasis grade,palliative gastrectomy combined with postoperative chemotherapy score,BI,PI and EI were independent risk factors for prognosis of gastric cancer patients with peritoneal metastasis,while ALB,RT,TTP and PNI were independent protective factors.The AUC of BI,PI,EI,RT,TTP and PNI combined predicted the prognosis was 0.909,which was significantly higher than that predicted by each indicator alone(P<0.05).Conclusion DCEUS parameters BI,PI,EI,RT,TTP and PNI have a certain relationship with the progression of gastric cancer with peritoneal metastasis.The combined prediction value of gastric cancer with peritoneal metastasis is reliable and has important clinical significance.
