Relationship between microinflammation and intestinal flora and diabetic nephropathy in elderly patients with type 2 diabetes mellitus
Objective To investigate the relationship between microinflammation and intestinal flora and diabetic nephropathy in elderly patients with type 2 diabetes mellitus(T2DM).Method A total of 154 elderly T2DM patients admitted to Guangyuan Central Hospital from January 2022 to December 2023 were selected as the study objects.According to whether diabetic nephropathy was complicated,they were divided into the the occurrence group(41 cases,with diabetic nephropathy)and the non-occurrence group(113 cases,without diabetic nephropathy).The microinflammation status(TNF-α,CRP,IL-6)and intestinal flora(Bifidobacterium,Enterococcus,Lactobacillus,Enterobacterium,B/E)in the two groups were compared and analyzed.To analyze the influencing factors of diabetic nephropathy in elderly patients with T2DM and the interaction between B/E and microinflammation status.Result The TNF-α,CRP,IL-6 and Enterobacterium in the occurrence group were higher than those in the non-occurrence group,while Bifidobacterium,Lactobacillus and B/E were lower than those in the non-occurrence group,with statistical significance(P<0.05).Multivariate logistic regression analysis showed that TNF-α,CRP and IL-6 were independent risk factors for diabetic nephropathy in elderly patients with T2DM(P<0.05),and B/E was independent protective factor for diabetic nephropathy in elderly patients with T2DM(P<0.05).The results of multiplicative interaction analysis showed that there was no multiplicative interaction between B/E and TNF-α,CRP and IL-6(P>0.05).The results of addition interaction analysis showed that the confidence interval of RERI and AP was>0,and the confidence interval of SI was>1,indicating that there was addition interaction between B/E and TNF-α,B/E and CRP,B/E and IL-6.Conclusion Elderly T2DM patients are accompanied by different degrees of microinflammation and intestinal flora disorders.TNF-α,CRP and IL-6 are independent risk factors for diabetic nephropathy in elderly T2DM patients,and B/E is independent protective factor for diabetic nephropathy in elderly T2DM patients.Moreover,there was additive interaction between B/E and microinflammation state.
