Application value of pelvic floor ultrasonic measurement of anterior chamber structure parameters in the diagnosis and prevention of postpartum stress urinary incontinence in primipara
Objective To investigate the value of anterior chamber structure parameters measured by pelvic floor ultrasound in the diagnosis and prevention of postpartum stress urinary incontinence(SUI)in primipara.Method A total of 160 primipara who gave birth in the Second Affiliated Hospital of Xinjiang Medical University from March to July 2020 were selected.According to the results of postpartum pelvic floor ultrasound review,patients with SUI were selected as the observation group(62 cases),and those without SUI were selected as the control group(98 cases).All the parturient women underwent pelvic floor ultrasound examination.The structural parameters of the anterior chamber,urethral rotation Angle,bladder descending position and bladder neck movement under maximum Valsalva movement were recorded in both groups,as well as the infundibular formation of the internal urethral orifice and cystocele before training.The movement of bladder neck and the changes of posterior vesicourethral Angle before and after 6 weeks of pelvic floor rehabilitation were compared.Result Detrusor muscle thickness,urethral inclination Angle and posterior vesicourethral Angle in the observation group were greater than those in the control group,while bladder position and bladder neck position were lower than those in the control group,with statistical significance(P<0.001).Under the maximum Valsalva movement,the urethral rotation Angle,bladder descending position and bladder neck movement in the observation group were greater than those in the control group,the differences were statistically significant(P<0.001).The rate of cystocele and the infundibular formation of the internal urethral orifice in the observation group before treatment were higher than those in the control group(P<0.05).The movement of vesicourethral posterior Angle and bladder neck after pelvic floor rehabilitation in the observation group was less than that before training,and the difference was statistically significant(P<0.001).Conclusion The structural parameters of the anterior chamber measured by ultrasound of the pelvic floor in primipara can well reflect the changes of the postpartum pelvic structure,which can be used as an objective basis for the diagnosis of postpartum SUI,and can also provide a reference for the formulation of pelvic floor rehabilitation program and prognosis assessment.
