Study on the Influencing Factors of Patient Hospitalization Costs Under DRG Reform——Taking EB19 Lung Major Surgery as an Example
Objective:The paper explores the changes in the cost structure and influencing factors of inpatients in the EB19 lung major surgery group after the reform of DRG payment methods,to provide a reference for the reasonable management of diagnostic and treatment costs. Methods:Retrospective analysis was used to analyze the medical record information of EB19 patients in the thoracic surgery department of a large tertiary hospital.Descriptive statistics, univariate analysis, and multiple linear regression models were used to analyze the influencing factors of patient hospitalization costs. Results:A total of 3734 patients were included in this study. The results of univariate analysis showed that gender, insurance type, age, length of stay, number of other diagnoses, number of other surgeries/procedures,and the use of robot-assisted operations all had an impact on hospitalization costs, with statistical differences among different groups(P<0.001).Spearman correlation analysis showed that the top three factors affecting hospitalization costs were consumables, surgery, and medication costs, with consumables having the most significant impact on hospitalization costs(P<0.001).Conclusion:In order to avoid the abuse of high-value consumables,it is necessary to continuously strengthen fund supervision, refine disease management, and achieve high-quality and sustainable development of departments and hospitals guided by valued diagnosis and treatment.
