目的 探讨与分析氧化锆个性化穿龈结构对薄龈生物型患者前牙区种植体周软硬组织的影响.方法 选取2019年5月到2023年8月在临沂市中医医院进行前牙区单牙种植修复的85例薄龈生物型患者作为研究对象,分为氧化锆组43例与金属组42例.所有患者都给予种植体修复治疗,金属组采用钴铬合金烤瓷冠辅助修复,氧化锆组采用氧化锆个性化穿龈结构辅助修复,记录患者种植体周软硬组织变化情况.结果 所有患者的种植体均成功存留,氧化锆组与金属组修复后1年的菌斑指数(PLI)显著低于修复前ld,牙周探诊深度(PD)显著高于修复前1d(P<0.05),氧化锆组修复后1年的PLI、PD与金属组相比也有差异(P<0.05).氧化锆组修复后3个月、修复后6个月、修复后1年的种植体边缘骨吸收量对比显著少于金属组(P<0.05).氧化锆组修复后1年的粉色美学评分(PES)、白色美学评分(WES)均显著高于金属组(P<0.05).氧化锆组修复后1年的牙龈炎、种植体松动、牙龈穿孔、瘘管形成等并发症发生率为2.33%,金属组为19.05%,氧化锆组与金属组相比显著降低(P<0.05).结论 氧化锆个性化穿龈结构在薄龈生物型患者的应用能改善前牙区种植体周软硬组织状况,提高种植体边缘骨吸收量,也能提高前牙区种植体的美学状况,还可降低并发症发生率.
The influence of zirconia customized transgingival structure on the soft and hard tissues around implants in patients with thin gingival biotype in the anterior tooth area
Objective To explore and analysis the influence of zirconia customized transgingival structure on the soft and hard tissues around implants in patients with thin gingival biotype in the anterior tooth area.Methods A total of 85 patients with single dental implant restoration in the anterior dental area from May 2019 to August 2023 were selected and divided into 43 in the zirconia group and 42 in the metal group.All patients were treated with implant repair,the metal group was assisted with cobalt-chromium alloy porcelain crown,and the zirconia group used personalized zirconia gingival structure to record the changes of soft and hard tissues around the implant.Results All patients'implants were successfully preserved,the plaque index(PLI)was significantly lower in the zirconia group and the metal group than 1d before repair,the periodontal probing depth(PD)was significantly higher than 1d before repair(P<0.05),and the PLI and PD of 1 year after repair in the zirconia group were also different compared with the metal group(P<0.05).The amount of bone resorption at the implant margin after 3 months,6 months and 1 year in the zirconia group was significantly less than that in the metal group(P<0.05).The pink aesthetic score(PES)and white aesthetic score(WES)were significantly higher in the zirconia group than in the metal group(P<0.05).The incidence of complications such as gingivitis,implant loosening,gingival perforation,and fistula formation was 2.33%in the zirconia group,19.05%in the metal group,and significantly lower in the zirconia group compared with the metal group(P<0.05).Conclusions The application of zirconia customized transgingival structure in patients with thin gingival biotype can improve the condition of soft and hard tissues around implants in the anterior tooth area,increase the amount of marginal bone resorption around the implants,enhance the aesthetic condition of implants in the anterior tooth area,and also reduce the incidence of complications.
