目的:分析腔内心电图(Intracavitaryelectrocardio—graphy,IC-ECG)定位技术辅助中心静脉通路装置导管(Peripherally Inserted Central Catheters,PICC)导管尖端定位的经济价值.方法:将2020年 1 月~2022 年 12 月本院收治的需要采取静脉PICC置管的患者40例作为本次观察对象,随机(红球与白球)分组,即对照组(常规PICC导管尖端定位,20例),观察组(IC-ECG定位技术辅助PICC导管尖端定位,20例),比较分析不同定位技术下一次性置管成功率、置管费用、操作时间、血流量、定位准确性、尖端位置正常率、并发症发生率(术后渗血、误穿、感染).结果:观察组定位准确性、一次性置管成功率、尖端位置正常率明显高于对照组,置管费用更少(P<0.05),但两组血流量、操作时间比较无明显差异(P>0.05);置管后并发症发生率对比可见,观察组更低(P<0.05).结论:IC-ECG定位技术辅助PICC导管尖端定位具有较高的经济价值,能减少置管费用,同时还能提高一次性成功率,并减少操作时间与血流量,较常规定位技术更能提高置管的安全性.
Study on the Economic Value of Intraluminal Electrocardiogram Localization Technology Assisting the Tip Localization of Central Venous Access Device Catheters(PICCs)
Objective:The economic value of ECG(Intracavitaryelectrocardio-graphy,IC-ECG)for the tip positioning of the catheter(Peripherally Inserted Central Catheters,PICC).Methods:A total of 40 patients admitted to our hospital from January 2020 to December 2022 who needed to undergo venous PICC catheterization were randomly divided into a control group(20 cases with conventional PICC catheter tip positioning)and an observation group(20 cases with IC-ECG assisted PICC catheter tip positioning).The success rate of one-time catheterization,catheterization cost,operation time,blood flow under different positioning techniques were compared and analyzed accuracy of positioning,normal rate of tip position,incidence of complications(postoperative bleeding,misplacement,infection).Results:The accuracy of positioning,success rate of one-time catheterization,and normal tip position in the observation group were significantly higher than those in the control group,with lower catheterization costs(P<0.05),but there was no significant difference in blood flow rate and operation time between the two groups(P>0.05);The complication rate was lower than the control group,statistically significant(P<0.05).Conclusion:IC-ECG positioning technology assists PICC catheter tip positioning has high economic value,can reduce the cost of catheterization,improve the one-time success rate,and reduce the operation time and blood flow,compared with the conventional positioning technology can improve the safety of catheterization.
