Objective:To study the diagnostic value of cerebral hemodynamic parameters of CT cerebral perfusion imaging(CTPI)in cerebral infarction(CI).Methods:A total of 50 patients with acute ischemic cerebrovascular disease who were admitted to our hospital from January 2020 to April 2022 were selected,and underwent CTPI examination.According to the results of cerebral MRI scan,the patients were divided into CI group(n=28)and transient ischemic attack(TIA)group(n=22).The differences of cerebral hemodynamic parameters between 2 groups were compared,and their diagnostic value for CI was analyzed.Pearson was used to analyze the correlation between cerebral hemodynamics parameters and the degree of nerve injury and severity of disease,and receiver operator characteristic curve was used to analyze the evaluation value of disease progression.Results:Cerebral blood volume(CBV)and cerebral blood flow(CBF)in CI group were lower than those in TIA group,while mean transit time(MTT)and time to peak(TTP)were higher than those in TIA group(P<0.05).The CTPI positive rate in CI group was higher than that in TIA group(P<0.05).CBV and CBF were negatively correlated with lesion size and National Institutes of Health Stroke Scale(NIHSS),while TTP and MTT were positively correlated with lesion size and NIHSS(P<0.05).The areas under the curve(AUCs)of CBV,CBF,TTP,MTT for the diagnosis of large CI were 0.804,0.831,0.78,0.756,and the AUCs for the diagnosis of severe neurological injury were 0.795,0.814,0.774,0.742,respectively.Conclusion:CTPI cerebral hemodynamic parameters can accurately diagnose CI,and are correlated with the degree of nerve injury and severity of disease,which can be used to evaluate the progression of cerebral infarction.
